- Evidence of house fly activity present: - Sticky traps were observed to be full- Fly carcasses were observed throughout the facility, on floors, and behind light fixtures.Evidence of mouse activity observed on-site:- Mouse droppings in drawers were observed in the kitchen area cabinets- A gap underneath the kitchen area sink was observed with mouse droppings near the gapCorrection:- Clean and sanitize mouse droppings in accordance with hantavirus mouse dropping protocol- Secure food products like flour in a pest-resistant container- Complete and retain the pest control checklist as part of pest control sanitation program- Secure the openings under the sink in a manner that prevents the entry of vermin into the kitchen area- Clean and sanitize areas with houseflies- Utilize a weekly, monthly cleaning schedule to ensure that excess garbage and places of harborage for pests are removed in a timely manner

- A coffee maker in the cupboard was observed with an old coffee filter inside its interior. A white-green mould-like substance was growing on the filter.A written sanitation program is requiredCorrected during inspection:- The filter was removed and the coffee maker was relocated to the sink area to be cleaned and sanitized.
