      • As you have had new cooking equipment installed under the commercial range hood, this is a good first step.BUTSince you have arranged the cooking equipment in the manner that you have, with a deep fryer between to pieces of equipment that have open flame ( stove top burners ) - this PHI does not think that this is a safe arrangement (of hot oil being near open flame).You will need to get a Safety Codes Officer to inspect and approve your cooking equipment AND provide a copy of the Safety Codes Officer report to your district PHI.

    • 26. Have plans for construction or alteration of this facility been submitted for approval?
      • PHI asked the staff person present about some new cooking equipment that had arrived at the store, but was not yet hooked up.The staff person reported that they are in the process of changing the menu to include East Indian food items and that there will be some equipment change over to accommodate the new menu items.PHI expressed that when there is going to be major pieces of equipment being changed out and a menu change of this nature - that it is important to keep the district PHI informed.Please ensure that a set of floor plans for the kitchen's new layout and a copy of the new menu is sent to your district PHI for review and approval.

      • There was a broken glass jar of pickles, being stored in the prep table cooler.PHI directed the staff member to discard this broken jar and its contents immediately, as there could be shards of glass in the product that cannot be seen.The staff member complied with this request.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • The kitchen hand wash sink was somewhat obstructed by a dirty pail in the sink, also there was no paper towel in the dispenser.The front hand wash sink was stocked and accessible.Corrective ActionPlease ensure that the kitchen hand wash sink is only used for hand washing purposes, and please ensure it is stocked with liquid soap AND paper towel, at all times.

    • 18. Are general nuisance conditions, other than those noted in other violations, being prevented at this food facility (Non-Critical)?
      • 1. At the janitor sink, the wall area under the faucet has become water damaged and mold is observed.2. In the corner to the south of the janitor sink, there is some old plumbing pipes jutting out from the wall. The 2 metal water supply lines are properly capped off with shut-off valves, BUT the 1.5 inch diameter black plastic drain line has not been properly capped off.3. In the utility room, there are a few missing ceiling tiles.Corrective Actions1. The damaged wall material will need to be removed (below the faucet ) and replaced with new material. Then an appropriate water proof material will need to be installed (example - tile), that can withstand the repeated splashing of water.2. For the identified plastic drain line, install a properly fitted cap to seal off this drain line.3. For the missing ceiling tiles, install ceiling tile materials to fill in most of the empty gap (note - there are electrical lines running through this area of ceiling, so keep the gap to a minimum due to these electrical lines).
