      • 3) Take out containers and paper bags on the shelves by the oven have rodent droppings on them
      • 4) Open spice containers have rodent droppings on them
      • Corrective Action(s): 1) Dough discarded at the time of inspection.
      • 2) Discard all pizza boxes that have been chewed and have rodent droppings on them. Pizza boxes must be stored in a pest proof container to ensure they do not become contaminated
      • 3) Discard all take out containers that are open and no longer in their factory sealed boxes
      • 4) Discard all spice containers that are not factory sealed

      • -The mixing including arm the holds the bowl, as well as the legs which have a significant amount of rodent droppings on it and was in use at the time of inspection
      • -The floor, platform and exterior of the floor buckets and bin as there is a significant amount of droppings as well as rodent stuck to a sticky trap

      • Corrective Action(s): Have a professional pest control company come in and eliminate the mice from the facility. Provide a copy of the pest control report.
      • Closure order WILL NOT be rescinded until the mice has been killed and removed from the restaurant.
