- 1. Lighting at some areas (food preparation and storage areas) appeared insufficient. Some light tubes were dim or burnt out. Please change the dim/burnt out tubes. 2. Some light fixtures at the food preparation area and dishwashing area had no protective covers. Please replace the missing covers.

- ** There was a missing ceiling tile at the oven/frying area. Please replace the missing ceiling tile.

- ** Some equipment at the facility were unclean. Dirt, debris and grease were observed on top of the dishwasher, oven and frier. Please ensure these equipment's and all equipment's in the facility are cleaned, sanitized and maintained in a sanitary condition.

- ** The facility requires some cleaning. Dirt and debris were observed at the following areas. - Underneath the dishwashing area, hard to reach areas under shelves at the food preparation areas, the corners at the back area of the facility, the wall around the dish pit/dishwashing area, the ceiling above the frier and back food preparation area including the ceiling vents. Please ensure these areas and the entire facility are cleaned.
