- Observation: When washing hands, no hot water was being dispensed by the faucet. Asked manager if there are issues with the water heater. During this time EHO was made aware that the unit had been replaced but was having issues and had stopped working. Manager was trying to re-light the tank but having issues.
- EHO made decision to issue a closure order, this was posted and signed by manager onsite. Staff had closed the facility and waiting for unit to be in working order.

- 2. Some of the sanitizer buckets observed with low concentration of Quat sanitizer.
- Corrective Action(s): 1. Instructed operator to fill one of the two compartments with Quat sanitizer and manually sanitize the dishes. Operator/staff will now manually sanitize the dishes until the dishwasher is fixed.
