      • *Repeat Violation*There was a gap between the two side doors. - Please repair the gap and ensure that the facility is pest proof.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • Food handling permit was not posted onsite. - Please ensure valid food permit is posted in a conspicuous place.

      • *Repeat Violation*There was a gap between the two side doors. - Please repair the gap and ensure that the facility is pest proof.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • Additional cleaning required in the following areas: - High-touch areas (fridge handles, sink taps, etc.)- Vents above the fryers- Shelves of and underneath the airdrying rack - Shelves in the front of the kitchen near the pizza oven. Please clean the facility including but not limited to these areas.

      • The paper towel was jammed in the dispenser and was not accessible. - Ensure hand sinks are stocked with soap and paper towel at all times.

      • There was a gap between the two side doors. - Please repair the gap and ensure that the facility is pest proof.

      • There was no sanitizer prepared onsite.- Please ensure that there is always sanitizer available onsite when food preparation is occurring.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • No test strips could be located onsite for the sanitizer used. - Please acquire appropriate test strips and ensure that solutions are tested regularly.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • The paper towel was jammed in the dispenser and was not accessible. - Ensure hand sinks are stocked with soap and paper towel at all times.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • There was some grease and dust buildup on the vent panels above the deep fryers. - Please clean the panels.

      • Observed staff not following proper handwashing procedures. Hands were just being dipped in the water with the detergent in it to wash them. - Ensure staff is following proper handwashing procedures.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • There was no paper towel at the hand sink. - Ensure hand sinks are stocked with soap and paper towel at all times.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • A buildup of food debris, grease, and grime were observed on the following areas:- Vents above the fryers and pizza oven- Floor under the fryer- Floor of the walk-in freezer To prevent pests and contamination, please clean and sanitize the mentioned areas.

      • Observed staff not following proper handwashing procedures. Hands were just being dipped in the water with the detergent in it to wash them. - Ensure staff is following proper handwashing procedures.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • The hand sink in the corner was blocked by a light. Spoke to staff who said that they did not use this sink to wash their hands and that they used their 3-compartment sink. The 3-compartment did not have soap and paper towels at it, and all 3 basins were also being used for other purposes. - Ensure staff have access to a stocked hand sink. Move soap and paper toels to 3-compartment sink and ensure one basin remains free for hanswashing. Or ensure the hand sink is in use and available.
