    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • The following services are no longer being offered: eyelash extensions, facials, eyelash perming. Please remove all of the cosmetic products and tools related to these services from the facility.Please notify this office if you wish to offer new services in the future.

      • Written procedures have not been completed.
    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • Written procedures do not meet the requirements set out in the standards.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Skin cleanser for body waxing is not available. Baby wipes will be obtained.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Numerous nail files observed at the manicure tables were in a used condition. These items are single-use and must be discarded after each use. Please discuss with staff.Re-iterated the requirement to discard single-use equipment such as foam/cardboard nail files, buffing blocks, toe separators, wooden handle foot file, cardboard covers for rotary nail drill bits etc.
    • 20. Is reusable equipment disinfected properly?
      • 1. Obtain 2 bottles of AHP7 indicator test strips.2. Maintain proper high-level disinfectant logs. Templates will be provided. Logs must be kept for each disinfectant tray used.3. Pedicure foot basins are not being properly disinfected. Operator advised they would buy PreEmpt RTU.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. There were unlabeled cosmetic products in various areas. Label all items/bottles/containers etc. with its contents throughout the facility and ensure original/bulk-supply containers are kept on-site.2. Dirty spoons/sticks were being left in cosmetic products at the pedicure stations which may contaminate the bulk supply.
