      • Observation: Droppings noted along back wall (water heater, stove, dishwasher and counter in this area).
      • Some appear older; however, there are some snap traps in this area as well.
      • Food debris in this area and garbage, moisture from the dishwasher.
      • No food storage in this area that is open.
      • Corrective Action(s): Clean and sanitize all along this wall. Remove all droppings.
      • Monitor if droppings reappear.
      • Store dish racks higher up to prevent contamination by droppings.
      • Contact a pest control contractor today, submit the report and actions taken to the Health Office.

      • 1. Boxes pilled up on top of coolers. Clutter build up in front of coolers and food storage that is not required for the premises.
      • 2. Cutting boards do not appear clean, have stains and food debris. Cooks have left for AM. No other staff available to clean after food prep.
      • 3. Equipment such as blenders, build up of utensils and mixing bowls on racks have spilled flour, dust, food debris build up.
      • 4. Food debris on floors underneath racks.
      • Corrective Action(s): Remove boxes and all items not required for food premises. (1 month)
      • Ensure that no clutter blocks food preparation; have all areas clean and clear, to allow for sanitary food preparation. (1 month)
      • Complete inventory of the racks area for utensil storage. (1 week)

      • Observation: Dishwasher contained 0 PPM chlorine residual at the plate surface. Unit was primed and still no sanitzer dispensing from unit.
      • Corrective Action(s): Repair so that there is a 50-100 PPM chlorine residual at the plate surface. Operator called in the dishwasher serviceman at time of inspection. Do not do any dishes until it is repaired. Monitor daily to ensure that it is functioning properly.
