      • Observed a jacket hanging on the shelving with prepackaged beverages and dry spices in the back kitchen area. - Personal items such as jackets must be stored separately from food and food equipment to prevent cross contamination. Corrected during inspection.

      • The 2-compartment sink is not draining properly (draining very slowly). Facility staff reported that the drainpipe is blocked at the grease trap joint, and a work order has been submitted to fix the problem. - The sink is not used currently. Dishes are being washed in the 3-compartment sink at the adjacent food facility that is owned by the same operator.

      • 1. No chlorine sanitizing solution was detected in the low temperature mechanical dishwasher. Facility staff ran the dishwasher 4 times, but no sanitizer was detected. 2. The wash water temperature in the dishwasher was 36C at the plate level, as measured by the PHI's thermometer.Advised facility staff to:- contact service provider for the dishwasher immediately to investigate the dishwasher. - wash and sanitize dishes manually until the dishwasher is repaired and verified to be functioning properly. - wash and sanitize all dishes that were washed previously in the dishwasher.Sept 16: Operator reported that the dishwasher service provider investigated the dishwasher and was not able to resolve the issue. The operator permanently opted to wash dishes manually in a 2-compartment sink (no reusable customer utensils are in use). - will install a 2-compartment sink and quats sanitizer dispenser at the sink within few days. Until this happens, operator will continue washing dishes in the 3-compartment sink at the next-door food facility (also owned by the operator).

Nov 1: Staff advised it was fixed but stopped working the next day. Staff advised they're cleaning dishes in the 3-compartment sink next door (Papa John's Pizza)
