      • Rodent feces were observed in basement dry storage area, and a dead rodent was found in a snap trap. These conditions present a contamination risk. Please ensure that the area is clean and sanitary.

      • The paper towel dispenser was empty at the front-end hand washing station. Staff replaced with new paper towel roll.

      • Mouse feces noted in the dry storage in the basement. Please ensure that the area is clean and sanitary.

      • There was a buildup of grime, grease or debris underneath the cookline and deep drying areas. Please clean these areas.

      • Mouse feces noted in the dry storage in the basement. Please ensure that the area is clean and sanitary. Mouse traps are present in the facility and checked regularly.

      • The ceiling plaster/paint was flaking off above the steam table. Please ensure that the ceiling is in repair and free of flaking materials to allow to for easier cleaning and prevent cross contamination.

      • The trim along the floor, in the mechanical dishwashing area, was missing. Please ensure that the wall to floor joints are sealed.
