      • **The violation still exists**Many repackaged bags of dry foods (nuts, candies, snacks, etc.) were not labelled with business name and contact information. Please label appropriately.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • **REPEATING VIOLATION**The long hot and cold water lines at the mop sink were missing vacuum breakers/back-flow prevention devices.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The area close to back entrance appeared dusty and dirty with food grime build up.-Ensure to clean and organize the area.
    • 24. Is solid and liquid waste being managed in a suitable manner?
      • 1. A wet, dirty mop was found stored in the two-compartment sink located at the back, which is designated for washing scoops.- Store mops only in the designated mop sink to prevent cross-contamination.2. The mop sink is currently installed close to the two-compartment sink without a splash guard, which poses a contamination risk.Relocate the mop sink to ensure adequate space and accessibility to the two-compartment sink.Install a splash guard or enclose the mop sink to prevent contamination.Do not store any food items, utensils, or equipment in front of or near the mop sink.

      • Paper towel dispenser in the staff bathroom was empty. Please restock current dispenser or install new dispenser for different sized paper towels.**Apr 7, 2021: roll of paper towels was not in a holder/dispenser.****Feb 1, 2024: paper towel dispenser was not dispensing/operating properly. Roll of paper towels on counter.
