Updated on January 22, 2026:* The pest control company was hired to conduct a pest control treatment (report was provided), and 3 more pest control treatments will be conducted. Please provide pest control reports to our office for review. Updated on February 9, 2026:* Received a report from Golden Arrow Pest Control company for the second cockroach's treatment - follow-up in 10-12 days was recommended.

      • The facility had a cockroach infestation: live cockroach was observed in the staff room. Ensure that a pest control company is hired to provide necessary pest control measures.       • No written procedures for skin cleansing prior to services, accidental cuts and reprocessing sink decontamination were available.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • The hand sink located in the waxing room was not equipped with the paper towels in an appropriate dispenser.

      • No product was available for cleansing a client’s skin prior to facial and body waxing services.

      • Single-use tools, including buffing blocks, nail files and toe separators were being reused. Ensure that single-use tools are discarded after each client.
