Updated on February 9, 2026:* Received a report from Golden Arrow Pest Control company for the second cockroach's treatment - follow-up in 10-12 days was recommended.

      • The facility had a cockroach infestation: live cockroach was observed in the staff room. Ensure that a pest control company is hired to provide necessary pest control measures.       • No written procedures for skin cleansing prior to services, accidental cuts and reprocessing sink decontamination were available.

      • The hand sink located in the waxing room was not equipped with the paper towels in an appropriate dispenser.
