      • A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED FOOD MANAGER WAS PRESENT WHILE POTENTIALLY HAZARDOUS FOODS (SOUP AND SANDIWCHES) WERE BEING PREPARED AND SERVED. ALSO, NO CITY SANITATION CERTIFICATE WAS POSTED IN THE ESTABLISHMENT. MANAGEMENT INSTRUCTED THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT AT ALL TIMES WITH THEIR ORIGINAL CERTIFICATE WHILE POTENTIALLY HAZARDOUS FOODS ARE PREPARED OR SERVED. SERIOUS VIOLATION 7-38-012.

      • The walls and ceilings shall be in good repair and easily cleaned. Observed small holes in wall behind 3-compartment sink. Must seal all holes in wall.
