      • Observation:
      • Cockroach & mice activity still observed in the premises. Cockroach activity observed under the ice making machine and around the desert chest freezer and prep sink.
      • Corrective Action(s):
      • - A thorough cleaning with hot soapy water of the premises is needed, especially the walls behind and under prep tables, sinks, & equipment; and the undersides of sinks & prep tables.
      • - Treatment of the whole premises, by a pest control company, for cockroaches shall be done. A 2nd treatment shall be done 10-14 days later.
      • - Forward pest reports/invoices to our office as proof that cockroach treatment is actively being done.

      • Observation:
      • Leaking drain pipes under the dishwashing spray sink and the 2-comp prep sink, to the left of the dishwasher. A bucket is catching the leaking waste water, and there is accumulated food & grease on the drain pipes. These conditions will encourage and breed pests (flies, cockroaches, mice).

      • - Contract a certified plumber to fix the drain pipes so they no longer leak.
      • - Ensure staff empty the bucket of water every night before going home, until the drain pipes are fixed.
      • - Wash the drain pipes to remove the food and grease.

      • Observation:
      • 1) Mice droppings and nesting materials found on the dry storage room shelves; on the shelving rack across from the dry storage room; and around the cupboard by the chest freezer storing the ice cream.
      • 2) A live cockroach was observed with a number of dead ones in the kitchen area and by the ice making machine. Owner stated they noticed the problem and had the premises treated (bait and spraying) 2 weeks ago.
      • Corrective Action(s):
      • 1) Clean up the droppings and nesting materials. Wash all areas with hot soapy water and then sanitize with bleach solution. Complete today.
      • 2) Empty all buckets & dish pans of liquid/water/food and wash them in the dishwasher, to remove all sources of food & water for pests. Complete today.
      • 3) Complete a thorough cleaning of the whole premises, especially under and behind equipment and tables in the cook line. Complete in 1 week.
      • 4) Have your professional pest company treat the premises at least 2 more times for cockroaches. Treatments should be about 10 days apart and done after a thorough cleaning has been done. Complete in 2 weeks.
      • 5) Have your professional pest company block any small spaces or openings at the base of the cupboard beside the ice cream freezer.

      • Observation:
      • 1) Bucket of liquid found in hot water tank room; waffle machines covered in grease in dry storage room; and dish in catching dripping pop syrup in the dry storage room; will are food and water sources for pests.
      • 2) Extra shelving racks stacked have nesting material on them. The extra shelving racks may have a space between each other that is housing pests.
      • Corrective Action(s):
      • 1) Retrain staff so they empty all buckets of water/liquid/syrup at the end of the night and wash the containers to eliminate any water/food source.

      • Observation:
      • 1) Accumulation of dust on ceiling tiles & upper walls in the kitchen area; on shelving racks; and on surfaces of equipment. The dust could fall on food and contaminate it.
      • 2) Food & grease debris, utensils and other debris found on walls and floors behind and under prep table, cooler and cooking equipment in the kitchen. This will attract and harbor pests.
      • Corrective Action(s):
      • 1) Wash the upper walls, racks, and ceiling with hot soapy water to remove the dust. Increase the frequency this is done to prevent the accumulation of dust in the future.

      • 2) A thorough cleaning of the walls and floors in the cookline area shall be done with hot soapy water and degreaser to remove the food debris and grease.       • Observation:
      • Found rodent droppings on shelves in the dry storage room, and on the shelving units & floor across from the dry storage room. Rodent droppings carry diseases and could contaminate food.
      • Corrective Action(s):
      • - Immediately clean up the rodent droppings and sanitize any surface that came into contact with the droppings. To clean, remove all items off the shelves first; then wash with hot, soapy water; then wipe down with hot water; and then spray a bleach sanitizer on the surfaces.
      • - Inform your pest company of the activity in these areas, and increase pest elimination efforts in these ares.
      • - Store all foods in food grade, hard plastic containers with tight fitting lids.

      • There is accumulated pop syrup on the floor in the dry storage room and there are packages of food (dried pasta and crackers) that rodents have tried to get into. These will attract and encourage rodents to remain.
      • Corrective Action(s):
      • 1) Immediately wash the floor with hot, soapy water. Ensure this is done every night before closing to remove spilt syrup and foods in the dry storage room.
      • 2) Purchase food grade, hard plastic containers with tight fitting lids to store all foods in, to prevent rodents from contaminating the food.

      • Observation:
      • 1) The door gaskets on the u/c cooler drawer, below the flat grills, are broken. Cold air is constantly escaping and the drawer slides open and could hurt staff.
      • 2) The "Tropicana" cooler is 7C and was storing butter and perishable items. The warm temperature will allow bacteria to grow in these foods and could make people sick when they eat it.
      • Corrective Action(s):
      • 1) Replace all damaged door gaskets on cooler drawers. Inspect all gaskets on cooler and freezer doors to ensure they are in good working order.

      • Observation:
      • Found mice droppings throughout the premises along the base of all walls; on lower shelves; and signs of gnawing (shredded material). Mice and their droppings carry diseases.
      • Corrective Action(s):
      • 1) Immediately clean the droppings up with hot water and soap. After washing, spray all surfaces with a sanitizer to kill any pathogens and to remove scent trails left by the mice.
      • 2) Block all openings and holes found, by tightly stuffing with steel wool and then covering with metal plates or wire mesh (1/4" or smaller).
      • 3) Every morning, staff MUST inspect the premises for mice droppings prior to starting cooking, and cleaning the droppings up.

      • 2) The door guard/sweep on the dry storage room is damaged, mice have gnawed the rubber at the base and entering the storage room.
      • Corrective Action(s):
      • 1) Block all holes and openings by tightly stuffing with steel wool; installing metal plates; and/or installing wire mesh (1/4" or smaller) over the openings.
      • 2) Replace the door guard/sweep on the dry storage room to exclude/prevent mice from entering the room.

      • 2) Dust accumulated on the ceiling tiles, air diffusers, light covers, and upper walls in the prep area of the kitchen. Dust can fall into food and contaminate it.
      • Corrective Action(s):
      • 1) Wash the mentioned areas with hot water and soap/degreaser, and then spray with a sanitizer to kill pathogens and mice scent trails. The mentioned areas shall be inspected and cleaned at least twice a day, once in the morning PRIOR to food processing AND at the end of the day prior to leaving.

      • Observation:

      • The following action must be taken:
      • > Immediately remove all mice droppings, and clean and sanitize all surfaces that have come into contact with the droppings.
      • > Contact your pest company and have them inspect the area above the walk-in-cooler and walk-in-freezer for rodents and entry points. Take appropriate action if activity or entry points are found.
      • > Increase the frequency the pest control company services the premises to eliminate the pest problem.
      • > Thorough and deep cleaning of the premises SHALL be done on a daily basis. The premises must be kept SUPER clean when there is a rodent problem.

      • 1) The exhaust canopies were due to be cleaned and serviced by a professional company in January 2023, as per the service sticker. Accumulation of grease in the chimney vent poses a fire hazard.
      • 2) There is accumulated food, grease, and debris along the base of all walls; on walls below and behind equipment, prep tables, moveable trollies, & shelving racks; on the exterior surfaces of equipment and their door handles; on cupboard doors and handles; and on legs of prep tables and other equipment.
      • Corrective Action(s):
      • 1) Immediately call an exhaust hood company and schedule a service and cleaning, to be completed by the end of this month.

      • 2) A thorough, deep cleaning of the mentioned areas require to be done immediately. Instruct staff to do the following:
      • > Use hot water and soap to scrub off the food, grease, and debris.
      • > Use hot water to rinse off the soap.

      • > Move racks, trollies, coolers, equipment, etc. away from the walls so the walls and floors beneath and behind can be cleaned.
      • > A thorough and deep cleaning shall be done EVERY night, until there no longer are any rodent problems.

      • 1) Repair or replace the drain pipe so it no longer leaks. Owner explained that a plumber has been called to repair the pipe.

      • ACTION: Block these areas with metal mesh (1/4" or smaller) or a metal plate to keep mice out. During the inspection, pest company arrived (scheduled date) to block holes.

      • ACTION: Scrub and wash with hot water, degreaser, and then spray with Bleach. This shall be done daily.

      • ACTION: 1. Repair/replace the counter tops so they are durable, smooth, level, smooth + sealed.
