      • Pest control records were not available at the time of the inspection. No signs of pest within the facility and traps were noted in several areas. Facility pest control is being conducted by facility maintenance. AHS pest control checklist sent to operator. ** Ensure pest control is conducted monthly, and records are retained on site.

A box of diced onions was noted being stored on the floor of the walk-in freezer. - Food items were stored on shelving units during the inspection- Ensure food is stored at least 6 inches off the floor at all times.
