- 1. There were large gaps between the stainless steel wall joints in the surrounding area of the dishwasher. 2. These gaps were repaired with caulking, but it was not sealed to render the surface as smooth, durable, easily cleanable, and impervious to moisture. The caulking on the left-hand side was covered in a pink slimy substance that appeared to be mold.Corrective action: The dishwasher and surrounding area is a high-traffic wet area, and therefore the unsealed caulking can absorb moisture and provide conditions that are conducive to mold growth. The damaged areas surrounding the dishwasher need to be repaired such that the surface is smooth, durable, easily cleanable, and impervious to moisture.

- An uncovered kettle containing water was sitting directly beneath the paper towel dispenser at the employee handwashing sink. As a result, the water for client service may become contaminated with water used for handwashing when an employee reaches up for paper towels. Corrective action: The kettle must no longer be stored directly beneath the paper towel dispenser. Additionally, the kettle should remain covered during periods of inactivity to protect its contents from any particulates or potential contaminants.

- Chlorine sanitizer solution in the low temperature dishwasher registered between 25 to 50 ppm with PHI's test strips. Corrective action: Please have your dishwasher serviced to ensure it can maintain a satisfactory chlorine sanitizer concentration of 100 ppm.

- The piece of finished flooring material in the kitchen, at the dough mixer - does not make a tight joint or seal to the surrounding floor material AND it is peeling up at the one corner.The finished floor material is to be smooth, sealed and impervious to moisture.Please have this identified area of flooring repaired.

On the cooking line there are 2 out of 10 bulbs burnt out, but all bulbs are covered.ACTION REQUIRED: 1. The bulbs appear to be fluorescent glass. 3. In the server area, there is a laminate counter area used to prepare hot water for beverages. The front panel of laminate is peeling off the wood counter beneath. This front panel and the perimeter around the counter has been tapped over with what appears to be duct tape. There is an odour coming from the exposed wood of the counter and the surface is damp.ACTION REQUIRED: 1. Please repair the drywall and paint so that the area is smooth, durable, non-absorbent, and easy to clean. COMPLETED2.
