- -The dishwasher was in disrepair. When the operator turned on the dishwasher, the water started pooling on the floor. The water came pouring down the dishwasher and the whole floor was flooded with water.The facility has a two-compartment sink to carry out manual dishwashing. Only disposable dishes must be used to serve food.

- -A dead deer mouse was observed in the mouse trap.-Pest control records were not available.Ensure pest control is done in the facility and records are available.
