- The interior pane of a rectangular window was observed to be cracked. Small shards of glass were observed on the floor and windowsill of the window.Correction:- Repair, replace or cover the cracked window in a manner that prevents further cracking and subsequent injury- Clean the area around the cracked window in a manner that removes sharp hazards

- The Black metal storage cabinet in the toddler space was observed to be top heavy and was no secured.Correction:- Relocate, Remove or Secure the black metal cabinet in a manner that eliminates tipping and crush injury hazards

- A multiuse climber was observed in the South East childcare area:- The climber was measured with a playable height of 49". Surfacing was not present in the indoor space for the climber.- Rope entrapment hazards were observed on the climbing net- Entanglement and strangulation hazard was observed on the top secured rope ladder. The rope ladder was unsecured at its baseCorrection:- Remove the structure from the care area- Notify PHI when the unit has been removed

- The Hand-sink in the out-of-school care area was not supplied with hand-soapCorrection:- Supply the hand-sink in the out-of-school care area with hand-soap

- The sanitizer solution cloth in the out-of-school care area was soiled with mud and was not in a usable condition.Correction:- Replace soiled cleaning cloths- Ensure that regularly surface cleaning is occurring in all care areas

- The following cleaning and general maintenance conditions were observed in the Out-of-School Area:- Foot-water marks were present from the exit door to the floor area near the hand-sink- The hand-sink was dirty with mud- Refuse, toys, and furniture items were scattered throughout the area.Correction:- Routinely clean and dispose of garbage and refuse throughout the day in the care area- Limit, Control, and manage slipping hazards as they occur- Remove excess toys, furniture, and learning supports from the play / care areas when no longer in use, clean, sanitize, organize or store items as required.
