- Observation: Mouse droppings noted on several surfaces in the coffee counter area. Notable behind and around Merychef, on some dried fruit containers and on the surface to the left of the glasswasher. There looks to be activity like the presence of insulation mixed with steel wool to the left of the glasswasher and cabinet corner space.
- Fruit flies still pretty evident on the walls and shelves in the coffee and pastry station. Fans seemed to have helped protect the pastries from fruit fly landings, although continue to monitor.
- Corrective Action(s): Staff need to make sure the entire counter surface in the coffee station is cleaned and sanitized daily. Staff should also be mindful of what droppings look like and clean and sanitize storage contianers daily so any droppings don't get to food products. Observations should be at the beginning of the day. There should be no droppings on work surfaces.

- Observation: A few food storage shelves in the kitchen observed with mouse droppings including:
- - pastry station at stand mixer shelf, and on shelf with mouse trap (showed operator);
- Floor drain grates could be cleaner;
- Some corners and cubby space around and behind cookline;
- Corrective Action(s): Make sure staff follow through with scheduled cleaning to address above or have added to cleaning schedule.
- Make sure mouse traps are not colocated on food storage shelves.

- Observation (CORRECTED DURING INSPECTION): Each and every sanitizer spray bottle measured 0 ppm quats. Measured 200 ppm at source with little priming required. It is more likely the issue of residual in spray bottles a function of poor rotation practices versus a priming issue at source.
- Corrective Action(s): I refilled all the spray bottles in facility, but would recommend it is a job function of one person for whole store and all spray bottles be dumped and filled per schedule by designated person.

- Observation: Mouse activity (droppings) noted in premises. Pest Control in monthly, but operator may be increasing frequency of visits to combat recent added activity.
- Corrective Action(s): Yes, increase frequency of Pest Control visits to bi-weekly or more until activity is reduced as discussed.

- Observation: Primary cookline area, especially below and behind equipment noted extremely dirty with accumulations of food debris observed. Also in the back there was an abundance of food debris and mouse droppings observed near and avround wooden chacoudary boards (stored on the floor). Dishpit was observed with mouse droppings on floor, stagnant water on containers on the floor, and with some food accumulations. Ceiling (exposed HVAC, lighting, and plumbing network) also noted with excessive accumulations of dust and debris that in sufficient accumulations to put food at risk of contamination.
- Corrective Action(s): A) A thorough cleaning reset is needed in the back of store, targeting the cookline, adjacent prep station and the dishpit.

- B) A cleaning plan and associated schedule is needed for the ceiling in the back area.
- **Target the timeline of cleaning for new year, but develop a plan to be included in sanitation plan for cleaning schedule as part of review on follow up inspection date noted.

- Observation (CORRECTED DURING INSPECTION): Multiple variety of pre-made sandwiches and quiches displayed at room temperature and intended for consumption (not including salmon sandwiches) in front retail area. Also noted multiple layers of eggs displayed on counter in bakery. Discussed with operator potential impact of extended time at room temperature will impact risk of these foods.
- Corrective Action(s): New quick oven to come into operation for ready-to-eat foods at retail station. Operational changes to include "display only" for all sandwiches/quiches etc. similar to salmon sandwiches to be implemented immediately.
