      • - The bleach sanitizer was stored on a corner shelf easily accessible to children, with a child sleeping adjacent to it.*This posed a safety risk, as the bleach bottle could potentially fall and the child could have easy access to the chemical.*Staff relocated the bleach sanitizer to the upper shelf, ensuring it is out of reach of children.

      • - Bingo colors (not safe for children) and paint glitters, labeled for use by children over 5, were found in classrooms #1 and #2.*Staff will review all art supplies, discard any unsafe materials, and replace them with non-toxic, age-appropriate options. Follow-up will be conducted.

Debris around the containers indicates these items are not being moved during diaper changing.REQUIREMENT: Ensure that there is sufficient storage space. If the shelving below is inconvenient for staff to store diapering items, install a shelf that allows for easier access while restricting access to children, and preventing items from being stored on the diaper changing surface.

Facility has bleach testing strips but is using a recipe on the spray bottles to make the solution. The bulk bleach used is more concentrated, meaning the recipe for 5% bleach needs to be altered.REQUIREMENT: If no rinsing step is to be done after spraying 500 ppm, then lower the concentration to 100 ppm. Verify the concentration of sanitizer using the available testing strips.
