      • Cupboard above the toilet is not locked and has air freshener stored inside it. Operator was instructed to store the air freshener on the top shelf so clients cannot easily access it if they opened the cupboards.

      • One ceiling tile is missing. Operator stated it was damaged.Ensure new ceiling tile is installed.
