    • 08. Is this facility properly constructed (Non-Critical)?
      • A small section of cubby/plywood used to secure the coat hooks in the OSC was not finished/painted. Please finish this area so that it is smooth and cleanable. Update: This has been completedJune 26/26One shelf in the toddler room is damaged. Remove, repair or replace the shelf by July 6, 2026.There were two ripped liners which were used to cover tables. One in the OSC and another in the preschool room. By July 6, 2026 ensure any damaged liners are removed.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Some toys and equipment throughout the program (ie. legos, large equipment, bins etc.) were observed to be dirty and require additional cleaning. JUNE 26/26 update: The OSC room was particularly dirty. Especially the front area with children's personal belonging are kept. Other areas in the OSc should be cleaner like large toys and the microwave. The other rooms had acceptable sanitation.Cleaning checklists were not being maintained daily. JUNE 26/26 update: the checklists are being maintained but in the case of the OSC it is not accurate,

      • Multiple examples of examples of personal belonging of two different children touching each other. By July 7, 2026 ensure there is adequate storage space so as personal items of kids is kept separate.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Some carpets were observed to be lifting and could become tripping hazards.

      • A small section of cubby/plywood used to secure the coat hooks in the OSC was not finished/painted. Please finish this area so that it is smooth and cleanable.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Some toys and equipment throughout the program (ie. legos, large equipment, bins etc.) were observed to be dirty and require additional cleaning.Cleaning checklists were not being maintained daily.

      • The first sink counter in the toddler/preschool bathroom was in disrepair with exposed wood. Please ensure the surface is smooth and cleanable, and impervious to water. Additional sealant is required around the basin and back of the counter to prevent water infiltration.

      • The faucet located at the small sink in the preschool bathroom was not properly operational. The same sink was not able to properly drain.

      • The bleach solution in the OSC was not properly labelled with its contents.

      • A broken baby doll was observed in the OSC. This was removed at the time of the inspection.A small trampoline was observed in the OSC. The protective edging was in disrepair. The trampoline was removed at the time of the inspection. Please note: AHS-EPH does not permit trampolines to be apart of regular daycare use. Only therapeutic use with a one-on-one staff and direct supervision is permitted.
