- Chlorine sanitizer was measured at 25 ppm in Kitchen. The operator prepared a fresh sanitizer solution, which was measured at 100 ppm.

- Expired chlorine test strips were observed. The operator was instructed to obtain valid test strips. The operator has since ordered new test strips and provided proof of purchase to the PHI.
