      • Ecopest document indicated that a pest monitoring inspection was conducted on May 7, 2025, with no pest activity observed.Action Required:Please continue to monitor monthly and send the pest reports.

      • Food handlers were not washing hands in between handling food and money.-Please ensure proper hand washing practices. Hands must be properly washed in between handling money and food, and after any other activity outside of food preparation.
    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • Food temperature not being monitored after frozen food were reheated.-Please ensure to monitor and document food temperature daily after reheating to assure that foods are reheated to proper and safe temperatures.

      • Mouse droppings observed behind the kitchen refrigerator.-Please clean and sanitize the indicated area. Monitor to ensure no further pest activity, if further mouse droppings are observed, please consult with pest control expert to prevent and stop all pest activity.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Chipped walls were observed in the facility's kitchen.-Please repair all damaged walls to ensure smooth, non-absorbent and easy-to-clean surfaces.

      • There is a hose which extends well into the basin of the dishwashing sink, creating a cross-connection risk.REQUIREMENT: Ensure that all plumbing, including fixtures, complies with applicable plumbing and building codes.
