- Observation: 1. Surfaces in equipment storage room are worn out and have build up of black matter. 2. Cardboard used to line shelves throughout the facility. Cardboard cannot be adequately cleaned and sanitized and can act as a reservoir for bacteria.
- Corrective Action: Remove cardboard and refinish surfaces to provide a durable easy to clean surface.

- Observation: Observed hole in wall in equipment storage room that was temporarily patched with cardboard.
- Corrective Action: Hole in wall needs to be patched appropriately along with any remaining holes beneath the sink.

- 209 - Food not protected from contamination [s. 12(a)]
- Observation: 1. Observed scoops stored inside food. Contamination of food from contact with objects, people, pests or chemicals can cause serious illness. 2. Observed cardboard used as lining throughout facility including inside drawers and on shelves. 3. Observed butter exposed to air inside cooler with black spots on top.
- Corrective Action: 1. Scoops are to be stored out of food in a sanitary manner and clean and sanitize them regularly. 2. Cardboard is to be removed and replaced with a material that is smooth, impervious to moisture, easy to clean and non-toxic. 3. Butter is to be discarded.
- 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
- Observation: No active sanitizer detected in sanitizer containers or spray bottles. Sanitizing solutions must be strong enough to destroy bacteria on surfaces to prevent the contamination of staff, clients or food on contact with the improperly cleaned surface.
- Corrective Action: Change sanitizing solutions as required to always maintain an effective concentration (200 ppm unscented bleach, 200 ppm QUAT)- follow manufacturer's specifications)
- 304 - Premises not free of pests [s. 26(a)]
- Observation: Evidence of rodents (e.g., mice) was present in that droppings were noted in under the kitchen sink. Rodents and other pests may contaminate foods and surfaces with feces, urine and saliva which may carry pathogenic organisms.
- Corrective Action: 1. Eliminate openings into the kitchen and food storage areas through which mice can enter (e.g., holes, gaps & cracks in walls & floors). 2. Area beneath the kitchen sink must be thoroughly cleaned and disinfected to remove rodent droppings.
