- 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
- 1. Refilled Lip Filler syringes were observed stored in cupboards from previous treatments and stored to be used on the same patients. Indicated that once opened, leftovers are to be discarded and cannot be stored. Requested that these be discarded and practice be ceased.2. Botox was observed being reconstituted with bacteriostatic saline solution when manufacturers instructions state to reconstitute with preservative free saline solution. One bottle was a week old and was requested to be discarded.3. Two vials of local anesthetic (Xlylocaine) were opened with no indication when they were opened and staff being unaware when opened. Instructions indicate a shelf life of 3 days once opened. Requested to be discarded.4. A large bottle of topical prescription numbing cream was observed to be expired.
