- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- The required written procedures were not observed. AHS written procedures template was provided by the PHI and adopted by the staff present during inspection.
- 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
- Single-use tools were being re-used. Used single-use nail files and buffers were observed at nail stations. The tools were discarded during inspection. Ensure used single-use tools are discarded immediately after use.
- 17. Is reusable equipment cleaned properly (Critical)?
- Some re-useable manicure tools such as nail clippers appeared unclean and rusty. The tools were discarded by the staff during inspection.Please ensure rusted or damaged tools are always replaced.
- 20. Is reusable equipment disinfected properly?
- Manufacturer's instructions for the use of high-level disinfectant (Preempt CS20) was not being followed. The staff stated that water is added to the solution before use.Education was provided by the PHI during inspection. Preempt CS20 cannot be diluted before use. Please ensure manufacturer's instructions are being followed.

- 1. High-level disinfectant (preempt CS20) at the facility had expired.2. Oxyvir TB disinfectant wipes in use at the facility had expired.3. Barbicide disinfectant bottle was almost empty.4. No disinfectant was observed for use on clippers. 5. High level disinfectant test strips were not observed.6. High-level disinfectant testing log sheet was not being maintained. Action items:1. Obtain a new bottle of high-level disinfectant.2. Discard the expired disinfectant wipes and obtain new ones for use.3. Obtain a new bottle of Barbicide. Also ensure a disinfectant jar is obtained for use.4. Obtain a disinfectant for clippers.5. Obtain high-level disinfectant test strips.6. Maintain high-level disinfectant testing log sheet. Poured high-level disinfectant must be tested each day re-used and log sheet maintained. Poured solution must be discarded when testing fails or according to the manufacturer's instructions.All disinfectants at the facility must be used according to the manufacturer's instructions including the contact time.
