      • QUAT was measured at a concentration of 100ppm from the dispenser. Please service the dispenser so it administers QUAT at a concentration of 200ppmOperators created a bleach water solution of 100ppm for disinfection until the QUAT dispenser is adjusted.

      • **Repeat violation**Preschool 3/Toddler 1 shared bathroom/diapering station: Diapering surface was visibly dirty. Diapering surface must be cleaned and disinfected with 100ppm bleach or 200ppm QUATS after every use.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • Preschool 3/Toddler 1 shared bathroom/diapering station:Miscellaneous items were stored on the diapering pad. - The operator reported that the diapering pad was not in use for that day. Please ensure to not store anything on the diapering station even when not in use as it poses a risk for cross contamination of the items and vice versa.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • A few of the electrical outlets in some of the rooms were missing protective covers. The inspector informed the operator that all electrical outlets that may be accessible to the clients must be protected at all times. The outlets were covered afterwards.
    • 06. Are proper protocols being followed in regards to client care?
      • The test strips for QUATs sanitizer solutions were expired. The inspector informed the operator that test strips must be used to confirm QUATs concentration.**OBTAIN QUATS TEST STRIPS.The facility was also using Chlorine as a sanitizing solution. There were no test strips on site for chlorine. If chlorine were to be used, please ensure to also obtain test strips.**OBTAIN TEST STRIPS FOR CHLORINE.
    • 06. Are proper protocols being followed in regards to client care?
      • The diapering change mat was not equipped with safety belts/latch in the infant room and the toddler room. The inspector informed the operator that diapering stations must be equipped with safety belts to prevent a child from falling off. This does not apply to diapering change mats that are placed on the floor.**EQUIP ALL DIAPERING MATS WITH SAFETY BELTS.

    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • The flow at multiple handwashing taps was significantly impaired so that handwashing has become ineffective. Measuring the maximum temperature was not possible at these taps. Rooms affected include:- Preschool rooms- Toddler rooms (interjoining bathroom child and diapering handwashing sink)REQUIREMENT: Repair or replace the handwashing taps and ensure the flow of water is not impaired.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • The facility has installed a dispenser for QUAT sanitizer; however, the dispenser is dispensing at a concentration to 500+ ppm.REQUIREMENT: Modify the dispenser so that QUAT sanitizer is dispensing at 200 ppm.
