      • There were two missing tiles observed on the bottom of the kitchen wall. Please ensure to repair the issue so that wall surface is smooth, durable, and easy to clean.The operator has confirmed that the missing tiles will be replaced by August 18, 2025.

      • The low-temperature dishwasher was tested for chlorine sanitizer, and it showed a concentration of 0 ppm. Staff were advised to make a chlorine sanitizer solution with a concentration of 100 ppm or a Quats sanitizer solution with a concentration of 200 ppm in the dishwashing sink to start sanitizing the dishes after washing them in the dishwasher, as well as to sanitize all dishes that has been cleaned previously by the dishwasher.The operator was advised to contact the dishwasher service provider immediately to get it fixed.

      • Personal belongings including bag, beverage, and slippers were found in the food preparation area and in dry storage room. Staff moved all belongings to the staffroom.A bike was also observed in the dry storage area. Staff mentioned that they will move the bike to space near the backdoor.Staff was advised to keep all their personal belongings in the staffroom.

      • The handwashing station at the entrance of the kitchen was found to be missing paper towels. Staff replaced the dispenser with paper towels during the inspection.

      • 1. One switch outlet next to the walk-in freezer was missing the cover. Manager mentioned that they will fix the issue immediately and inform the PHI when done.

      • The panko container had scoops that were in contact with the breadcrumbs. Staff was advised to remove the scoop and store it in a separate container.

      • A spray bottle of quat sanitizing solution was not labeled in the kitchen. Corrected at the time of inspection.

      • The refrigerator for dressings was maintaining food at 10C-14C. The temperature gauge on the refrigerator was reading at 49F. - Advised kitchen staff to transfer high-risk foods such as creamer, milk, tomato juice, and pancake sauce to another refrigerator. Facility staff transferred these high-risk foods to the other refrigerator that maintains satisfactory temperature at the time of inspection. - Please ensure the refrigerator is serviced/adjusted to maintain satisfactory temperature (4C/40F or coder).

      • Noted plastic cup without handle stored in bulk food container in direct contact with stafford chicken in the dry food storage room. Facility staff removed the cup at the time of inspection. Please use scoops with handles and store scoops in sanitary manner to prevent contamination of food.
    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • Observed 41 raw eggs stored on a counter at room temperature. Eggs measured at 19C to 23C with infrared thermometer. Facility staff were in doubt how long the eggs were left out of temperature control. All 41 eggs were discarded into a garbage container at the time of inspection. Please store eggs in coolers to ensure they are maintained at 7C or colder.

      • Noted the exterior door leading to the patio prompted open and the interior screen door not closed completely - allowing entry of pest. Operator closed the exterior door at the time of inspection. Please fix the screen door to self-close properly or do not leave the exterior door prompted open.

      • Noted 3 dish racks that were not maintained in clean and sanitary manner. Operator advised the racks are not cleanable anymore and new racks were ordered to replace.
