    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • OSC Room: Wood glue was found in an unlocked cupboard, labeled with a warning sign stating “Keep out of reach of children.” The operator was advised to remove it and ensure it is stored out of children’s reach to maintain safety.
    • 06. Are proper protocols being followed in regards to client care?
      • Baby room: Chlorine sanitizer was measured at 50 ppm. The operator was advised to prepare fresh sanitizer to achieve the required concentration of 100 ppm to ensure effective sanitization.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Pre-School shared washroom: One hand dryer was observed to be non-functional and unplugged, as it was not shutting off automatically. Children were observed washing their hands and then returning directly to the room without drying them. The operator was advised to provide paper towels as a temporary hand drying option until the hand dryer is repaired and fully operational.

      • OSC Room: First aid kit items, such as cold packs, were found to be expired (expiration: 2024). The operator was advised to regularly monitor and inspect first aid kits to ensure that all contents are within their expiration dates and remain in good condition for effective use during emergencies.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • OSC room and Pre-school Room: The couch was found to be in an unsanitary and deteriorated condition. The surface shows significant wear, including visible cracking, discoloration, and dark staining embedded in the upholstery. The material appears heavily soiled and is not able to be effectively cleaned or sanitized due to the damaged surface.The operator was advised to remove and replace the couch with a new one.

      • Repeated violation: 2026-06-18: Storage between clients personal items is not adequate. 2025-10-22: Storage between clients personal items is not adequate and is touching in each room. Either space hooks further apart, or install separators between clients personal items.

      • Toddler 1 Room: Unlabeled water bottles were observed stored together in a shared container. Staff were advised to clearly label all bottles and items for each child to ensure proper identification and to maintain children’s health.

      • The handwashing basin is not directly adjacent to the diapering area.Diapering change table to be moved.

      • Some of the hand sinks in the facility are not draining properly.Sinks included were Baby Toom, Pre-K 1, and toddler. Ensure that hand sinks are draining properly.

      • Bingo dabbers that were not labeled as non-toxic were available in some of the client rooms. These were removed from use. Ensure that all art supplies being used as labeled as non-toxic.

      • *Push pins were observed to be used for displaying art in care rooms. Push pins may become a hazard if not secured in place. Please ensure children art is display in a safe matter.

      • Safe locks in the baby room were observed to be in disrepair. Ensure cupboards/drawers are secured for child safety.

      • Diapering mattress was noted to be in disrepair (holes were observed). Ensure all areas for diapering are easily cleanable and sanitizable, impervious to moisture.Sleeping mattresses were observed to be in disrepair around corners. Ensure sleeping mattresses are easily cleanable and impervious to moisture.

      • *Sanitizer solution was observed to be under 100 ppm (measured with test strips). This was corrected during inspection.*Air intakes were observed to be dusty. Ceiling tiles around them were dusty as well. Ensure areas are in good sanitation.

      • *Bingo dabbers with label " keep out of reach of children" and no AP sign were observed. This was removed from use. Ensure art supplies are non-toxic and safe for children use.

      • *Safety locks for one cupboard drawer in disrepair. This was repaired immediately by the operator.
    • 08. Is this facility properly constructed (Non-Critical)?
      • *Walls in disrepair. Exposed dry wall observed. Ensure all surfaces are smooth, impervious to moisture and easily cleanable.*Black table in disrepair (exposed wood observed). Ensure all equipment is in good repair and easily cleanable.

      • *Ensure first aid kit is supplied with required equipment. Low first aid kit supplies observed.

      • *Low lighting in the room. Ensure adequate lighting is available to ensure a good environment for cleaning and disinfection.

      • *Diapering mattress in disrepair. Holes in the mattress were observed. Ensure diapering table is in good repair.

      • *Play sand appears to be needing cleaning. Debris were observed. Ensure sand is changed/cleaned regularly.*Play sand table in disrepair. Tape was observed on the play sand table lid.

      • *Sleeping mattresses appears to be cleaned once a week as per the operator of one of the rooms. Sleeping mattresses were observed to be stored together (ie. stacked). Ensure sleeping mattresses are disinfected on a daily basis, in between use. Cleaning schedules not observed.

      • * Electrical plug was observed to be covered with tape. Ensure electrical plugs are safely maintained.

      • *Labelled clean face cloths were stored on the hooks in the close proximity of the diapering area. this practice is discouraged due to the possibility of cross contamination.*Used cleaning cloths observed to be stored on different surfaces. Ensure cleaning cloths are adequately managed. *Ensure sanitizer is stored in a bottle that show the content of the bottle. Improperly labeled bottle observed.

    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The door/gate between preschool 1 & 2 (via the washroom) was broken and removed. 2. The door/gate between toddler's room 1 & 2 was not a bit loose and not locking properly.The operator stated that repairs were already scheduled. Please ensure the required repairs are completed.

      • The handwashing sink used for diapering in the infant room was being used to wash bottles. Please note that a handwashing sink designated for diapering must be used for only handwashing. Toys can be washed and disinfected at a separate sink. The kitchen sink can be used.
