      • 1. There were a few floor tiles that were chipped on the corners.2. A large section of the floor in front of the dishwasher was missing tiles, and was being covered with carpet and duct tape, which is absorbent and not easy to clean. **April 9. 2025: carpet and duct tape removed, and a rubber mat was placed on top of this area. However, there is unfinished wood and concrete floor in this area now, which is not easy to clean and maintain.COMPLETE THE FOLLOWING:1. Repair flooring so that it is in good repair and easy to clean and maintain.**Renovations planned for July 2025**

    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • 1. The dishwasher was not able to fill with water, as it was connected to the same water line as the handwash sink. COMPLETE THE FOLLOWING:1. Conduct the necessary repairs to ensure the dishwasher has a constant water supply and is able to wash and sanitize dishes.2. Until the dishwasher is repaired, manually wash dishes in the 3-compartment sink.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • 1. While checking for hot water at the handwash sink, the water was cutoff and there was no running hot water available. 2. No paper towel in a holder/dispenser at the handwash sink.COMPLETE THE FOLLOWING:1. Ensure hot running water is returned to the handwash sink and that it is always available.2. Replace paper towel and store in a holder/dispenser.
