      • 1. Bite marks were noted on soft toys. These toys were discarded during the inspection. 2. A few soft blocks were noted to be damaged. Please repair these blocks. 3. It was noted toilet paper rolls were being used for arts and crafts. Please do not use toilet paper rolls for arts and crafts.

      • 1. One of the washrooms sink in the middle room upstairs was not in operational condition. The operator indicated that the plumbing line has been leaking. Please repair the leaking plumbing line. 2. The water temperature was noted to be between 26 - 34 degrees C. Please maintain water temperature between 35 - 45 degrees C.

Please investigate this concern and ensure that heating facilities are in operational state and are capable to maintaining heat at or above 20 degrees C.

      • 1. Three wiping cloths were noted to be stored on diapering station located in the baby room. Please do not use diapering area for storage area. Please properly cleaned and disinfect the diapering area.2. It was noted that the facility was not disinfecting the diapering station properly. Please ensure that all diapering stations are properly soaked with 1000 ppm bleach solution to achieve proper disinfection levels. 3. It was noted that facility was using cloth towels to wipe soiled diaper stations. All used cloth towels were being stored with wiping cloths that are being used for disinfecting other areas such as tables, food areas, and cleaning clients faces. Please do not use cloth towels to wipe soiled diaper stations. Please use paper towels to wipe soiled diapering stations.4. It was noted that the staff members were not following proper diapering procedures. Staff members were educated with proper procedures during the inspection.

      • 1. Water damage was noted on the ceiling tiles located by the emergency exit in the large room in the basement, staff kitchen and storage room in the pre-school area. Please investigate the source of the water infiltration and repair it. Please replace all damaged tiles. 2. A hole is present in the wall behind the toilet bowl in the panda room washroom. Please seal this hole.3. Wall tiles were noted to be missing from the boy's washroom in OSC area. Please install the missing wall tiles.

      • Duct tape was noted on the drain of the sand table. Please remove the duct tape.

Please increase the water temperature and ensure it is ideally maintained between 35 - 45 degrees C.2. The maximum water temperature was noted to be 31.3 and 31.6 degrees C at both washroom sinks located in the joey room.

Basment room was not following toy washing procedures. Proper toy washing procedures were discussed with the staff member during the inspection.

      • 1. One client was noted to be sleeping on the floor in the pre-school room next to three tires.Please ensure each client is provided with a bed. Please do not let clients sleep on the floor. Please do not stack tires on top of each other. 2. Mattress sheets were noted to be missing from few beds. Please ensure sheets are present for all beds/mattresses.

      • 1. Toilet bowl not in operational state in Joey's room. Please repair the toilet bowl. 2. One of the handwashing sinks located in the washroom of the panda rooms was not draining.Please repair the plumbing line.

      • 1. Buildup of dirt was observed in the baseboards located in one of the washrooms. -Baseboard was in disrepair. Program indicated that the landlord was following up with repair or replacement.

Please refinish all surfaces and ensure it is maintained in a smooth, easy-to-clean and durable state. FEB 8 and 23: Walls were in process of being patched. Discussed timelines for painting walls in child care rooms. 2. Furniture items noted throughout the facility are no longer smooth and easy-to-clean. Please refinish all furniture items.

      • General cleaning and organizing had improved. *Deep cleaning of cupboards and baseboards was noted to be required with a build up of dirt observed.

      • 1. Buildup of dirt was observed in the baseboards located in one of the washrooms. 2. The hand washing sink was leaking in one of the washroom. Please repair the leaking sink.

      • The window coverings pull cords are not secure and can cause strangulation hazards. Please properly secure all window covering pull cords. *Discussed on site with Director regarding securing window pull cords or ensuring the cords are kept out of reach of children.

      • Diapering was observed at time of inspection without diapering safety strap.Safety strap was repositioned and staff reminded to use the diapering safety strap for each diaper change.

      • Toy washing was discussed - all toy washing is to be completed in kitchen sinks (when dishes and food preparation are not occurring). * Ensure all toys are cleaned and disinfected on a weekly schedule with the 3 sink method. Mouthed toys to be put in mouthed toy bin until cleaned and disinfected.

      • 1. The maximum water temperature of the water was noted to be at 53 degrees C.       • Washrooms in the facility requires cleaning. Please clean and disinfect all washrooms thoroughly.

      • Hand hygiene is not being performed. Proper glove policy was not being followed with. Importance of hand washing, and proper glove policy were discussed with the operator during the inspection. Hands must be washed in between tasks. Please wash hands before putting gloves on, after taking them off, and between tasks. Gloves must be replaced between tasks.
