    • 06. Are proper protocols being followed in regards to client care?
      • Test strips present were expired. Expired test strips may give inaccurate results. Please obtain new test strips that read up to 1000 ppm.
    • 08. Is this facility properly constructed (Non-Critical)?
      • The wall behind the sink of cozy b has had the coat rack removed, leaving holes in the wall that water can seep into which could lead to mold growth. Please properly repair the walls and ensure they are smooth, non-absorbent and easily cleanable.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towels are not being consistently replaced in the paper towel holder where diaper changing occurs as staff can't easily access the key. Ensure the paper towel is replaced in the holder when emptied to ensure proper hand washing after diapering can be done.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • The hot and cold taps are reversed at the hand sink in cozy b which could lead to accidental scalding. Please ensure the hot and cold taps are properly connected.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • One training potty was left with urine in after use and had not been emptied, cleaned and disinfected. Operator cleaned and disinfected during the inspection. Ensure all caregivers are properly cleaning potties after use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • 1. The fridge used to store minimal food items or medications (none at this time) was not working and the ambient temperature was over 20 C. 2. Sanitizer is being mixed to 1000 ppm when only a 100 ppm solution is required. Please have the fridge repaired or replaced and ensure surfaces and toys are being disinfected with a 100 ppm solution . 1000 ppm solutions should be used during outbreaks.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • The furnace room and chemical storage room were not inaccessible to children. The furnace room has no means of locking or securing the door and the chemical room has a latch that was not in use allowing the door to open freely. All rooms that contain hazards must be kept inaccessible to children at all times.
    • 05. Are communicable diseases at this facility being properly managed?
      • Staff indicated body fluids such as vomit are cleaned with a 100 ppm bleach solution. Body fluids are not being disinfected with an appropriate level of disinfectant. Please ensure all staff are aware of proper concentrations for cleaning of blood and body fluids. Concentration levels are listed in Appendix I of the Health and Safety Guide for Operators of Childcare Facilities.
    • 06. Are proper protocols being followed in regards to client care?
      • No disinfectant was present in the diapering room with the diaper change table. All rooms where diapering occur must be supplied with an appropriate disinfectant for use. Please ensure a bottle of disinfectant is in each diapering room and stored out of reach.
    • 08. Is this facility properly constructed (Non-Critical)?
      • There is a broken shelf in Cozy Room B that is leaning against the wall and could lead to injury of a child. Please have the shelf removed or reinstalled to be safe and of sound construction.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towels in the diapering area were not in the dispenser. Please ensure paper towels are stored in a manner that protects them from contamination.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The diaper change pad surface is broken in two places exposing the foam filling beneath. Non-porous surfaces cannot be cleaned and disinfected properly and can harbor bacteria. Have the change pad replaced with one in good condition.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Chemicals are stored in a separate room at the end of a hallway where children's rooms are. One child was observed wandering into the hallway near the chemical storage room and the room was not locked at the time. There is a latch to prevent entry and the latch was then attached. Ensure the chemical storage room is always locked to prevent anyone accessing chemicals.
    • 06. Are proper protocols being followed in regards to client care?
      • Diapering of children was observed with staff sitting on the bathroom floor and changing one child's diaper, then starting to change another without disinfecting the area or changing gloves. No sanitizer was present in the room for use before changing of diapers occurred. The diaper change table had rocks and crayons and crumbs observed, indicating it is not being properly cleaned and disinfected. Surfaces in the change room must be disinfected between children.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towel was not present at the hand sink in the preschool room. Paper towel was not in dispensers in the washrooms. Please ensure paper towel is provided in dispensers at all times at all hand sinks.
    • 11. Are parent provided foods being properly handled and stored to ensure the safety of clients in this social care facility?
      • A bottle of formula was observed without a label to indicate which child it belongs to. Please ensure all formula is labeled correctly.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Children's blankets for nap time in the preschool room were being stored together in cupboards with no separation. Please provide bins that are labelled for each child's belongings.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Diapering procedure observed involved staff placing gloves on their hands without washing them, sitting on the floor of the bathroom, changing a diaper and then with the same gloves, holding another child's hand, then starting to change another child's diaper without changing gloves or washing hands. Unwashed hands and gloves can cross contaminate surfaces and transfer disease from person to person. Please ensure hands are washed before and after glove use, and gloves are changed between changing one child's diaper and another.

    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • Temperature regulators are only installed on some of the sinks children have access to. Temperature of water in mens' washroom was 55 C. Operator indicated that the hot water was turned down at the hot water tank. Hot water tanks are to be kept at 60 C to prevent legionnaires from developing. Please ensure the hot water tank is set to 60 C and temperature regulators are installed on all sinks that children have access to.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Bottles containing bleach did not have the contents written on the label. Please ensure all chemicals are properly labelled.
