      • Two tall shelves are not anchored with the wall. The operator was advised to securely anchor the shelves to the wall to prevent movement and reduce the risk of items falling or injury. Some carpet edges were observed lifted, posing a potential trip hazard. The operator was advised to securely attach the carpet edges to the floor to prevent injury.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Two bleach solution spray bottles are provided to the diaper change stations. Onsite employees are sure about the difference of these solutions. Owner later indicated that these solutions have different concentration, one for diaper change tables and other for sanitization of other surfaces. Operator is advised to educate the onsite employees for proper use of chemicals. Operator is also advised not to store chemicals at diaper change stations which are not required for this station.

      • Hot water is not observed at the hand wash sink in toddler room. Operator is advised to immediately provide hot water supply to this hand wash sink.

      • Diaper change pad is damaged. Operator removed it at the time of inspection.
