      • Observation: No chlorine detected during the final rinse cycle of the dishwasher at the time of inspection. No sanitizer monitoring logs available at the time of inspection. Operator called the dishwasher repair person during the inspection.
      • Corrective Action(s): 1. Ensure that dishwasher has at least 50 ppm chlorine during the final rinse cycle. ( immediately)
      • 2. Untill the dishwasher gets repaired, you must either use disposable dishes or sanitize the washed dishes by soaking in 100 ppm chlorine solution ( 1 ounce of bleach per gallon of water).

      • Observation: Mouse droppings noticed throughout the premises with heavy activity noticed behind the deep freezers in dry storage area and behind the cookline and tempura station. Noticed pieces of chewed through insulation behind the cookline.

      • 1. Hire a licensed pest control company to initiate mice monitoring and control. Identify all hot spots and start control measures immediately.
      • 2. Clean and disinfect all areas and surfaces affected to remove mice droppings. Clean all equipment, shelving, tables and undernath the dishwasher areas. All equipment must be removed to clean areas underneath and behind..
      • 3. Ensure to keep all food items in bins with tight fitting lids overnight.
      • 4. Fix/repair any leaky faucets.

      • Observation: Poor sanitary conditions noted throughhout the restaurant with build up of grease and food debris especially in the dry storage areas, underneath and behind the cookline, uprightt coolers and preparation coolers
      • Corrective Action(s): -Thorough cleaning is required for the entire kitchen and sush prep areas to remove food and grease build up. Ensure to clean the floor-wall junctions thoroughly to remove all build-up. Ensure to clean the wheels for all equipment and legs for all tables.
      • -Daily inspection and cleaning of the above areas is required to elliminate food sources for mice. ( By May 13th).

      • Observation: Extensive food debris and grease build up noticed underneath freezers, fridges, sinks and other equipment as well as on walls.
      • Corrective Action(s): Thorough cleaning required to remove all food debris, caked on grease and food spills from all areas. This inlcude
      • dry storage area, main kitchen, sushi bar area and staff washroom. You must purchase and use commercial grade floor cleaner and degreaser to clean effectively. Floors were found to be too greasy and slippery at the time of inspection. ( By July 30, 2021).

      • Observation: 1. Dishwasher final rinse @25 ppm chlorine after 2-3 runs.
      • 2. Heavy grease and soot build-up noticed on the kitchen hood canopy with filers being very greasy.
      • Corrective Action(s): 1.Repair or adjust the dishwasher so that it is capable of dispensing at least 50 ppm chlorine at the plate surface at all times,
      • You must monitor and record dishwasher chlorine levels every day. ( Immediatley).
      • 2. Hire a professional vent cleaning company for cleaning the hoods. All filters must be cleaned at least every every week

      • -Behind the deep freezrs in the dry storage room.
      • -In between the line cooler and side prep table ( they were joined together with aluminum tape which was completely covered with caked on food debris
      • -The side walls of the line cooler across from the grill
      • -underneath the double compartment food prep sink and in the sink backsplash area
      • -Underneath and behind all coolers
      • -Dry food debris noticed in the utensils stored above the line cooler
      • -Majority of the ceiling tiles are covered with soot and smoke.
      • Corrective Action(s): Clean and sanitize all of the abve equipment and surfaces by February 10, 2020.

      • Observation: -One of the rice cookers had paint peeled off in the cooking bowl's surface
      • - A small white cutting board was found to be deteriorated by knife cut marks making it hard to clean.
      • Corrective Action(s): The rice cooker's cookign bowl was discarded during the inspection.
      • The cutting board was also discarded.You must purchase a new commercial rice cooker and a couple of new plastic cutting boards ( Immediately).

      • Observation: 1. Heavy grease bulid up noticed on the grease filters in the fume hood.
      • 2. The dishwasher not reaching high enough temperature for adequate washing of dishes. It reached 34.5 and 38 C during two consecutive cycles. Besides, it is not rinsing the dish detergent off the dishes.
      • Corrective Action(s): 1. Professional cleaning and servicing through a vent cleaning company requried by Feb. 13, 2020.
      • The grease filters must be removed and cleaned using a degreaser every TWO WEEKS.
