      • Observation (CORRECTED DURING INSPECTION): REPEAT VIOLATION: Front cooler unit to display foods found to be at 21C on the unit thermometer. Foods being prepared at the time of inspection.
      • Corrective Action(s): -Cooler door was closed and cooler unit achieved <4C during inspection. The previous Environmental Health Officer had noted this exact same issue and provided instruction to ensure foods are stored at 4C or below in the coolers. You are required to keep this cooler unit on, doors closed, and at 4 degrees C or below. If this issue is noted again, facility will face enforcement action including ticket(s) and/or Health Act Orders.

      • Observation (CORRECTED DURING INSPECTION): Both display coolers observed wide open and measured at 20'C during inspection. Staff kept doors closed for about 20 minutes, and temperature of both coolers dropped to 4'C or lower.
      • Corrective Action(s): DO NOT keep the doors of any coolers open during preparation. All doors must be kept closed and not frequently opened to prevent temperature fluctuations, time-temperature abuse of food and foodborne illness. All potentially hazardous food must be stored at or below 4'C at all times.

      • Observation: A few mouse droppings observed on floor by the washroom and dishwashing sink. Professional pest control company contracted. Traps in place.

    • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): 1) No sanitizer solution available for food prep surface. 200ppm chlorine solution was prepared in a spray bottle at the time of the inspection. Staff was advised to have all food prep surfaces sanitized using this solution.
      • 2) Manual dishwashing being done without the sanitizing step. When asked, dishwashing staff was unaware of the proper way of manual dishwashing and could not locate the sink plug. Another kitchen staff had located the sink plug, which was stored in the upper cabinet.
      • Corrective Action(s): Ensure to have 200ppm solution prepared and available for use at all times. Have all food prep surfaces sanitized before initiating any food prep in the morning, after each possible contamination and every 2 hours during operation.
      • Ensure to follow the proper way of manual dishwashing: Wash with soap/warm water (45'C) --> Rinse with warm water (45'C) --> Sanitize by soaking in 200ppm chlorine solution for at least 30 sec --> Air dry.

    • 212 - Operator has not provided acceptable written food handling procedures [s. 23]
      • Observation: Facility was bringing cooked food from another location (Kim Bab Heaven) on North Rd every morning before opening.
      • Corrective Action(s): Ensure to submit the following information to the health inspector:
      • - list of all items being prepared at Kim Bab Heaven
      • - method of transportation/ at what temperature are the foods being transported to Sushi Tree
      • - delivery schedule (from Kim Bab Heaven to Sushi Tree)
      • - storage method on site (before assembly on site/after assembly in display cooler at what temperature)

      • Observation: Small number of dried/old rodent droppings observed on floor underneath dishwashing area.
      • Corrective Action(s): Ensure to have all droppings removed and affected areas sanitized using 1000ppm (1:50) chlorine solution. Fill the hole as mentioned under Code305 and continue to monitor the situation.

      • Observation: Rice observed on floor along the edge of the wall by the dishwashing area. A small hole observed on wall by the dishwashing area.
      • Corrective Action(s): Ensure to clean floors on a daily basis as per facility's Sanitation Plan. Do not leave any food debris on floor overnight.
