    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • -One of the handwashing stations, in the room to the right of the entrance, was leaking water when in its fully off position.**Please repair the faucet so water is no longer leaking and ensure the hand washing station is fully operational. -
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • -The fans in the washrooms were not operating properly as it did not pass the toilet paper test. ** Please repair or replace so the fans are operating properly. To test: place a toilet paper to the fan, and if it is able to be held up, it indicates the fan is operating as intended and there is adequate suction.-

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • - 1. The spray bottle containing bleach mixed with water was not labelled to indicate it's contents.**Please label the bottle to indicate its contents** **This was corrected during the inspection**2. A can of lysol spray was observed in an unlocked cabinet near the handwashing stations, accessible to children. **Please relocate chemicals so they are inaccessible to children or install child-safety locks so the cabinet cannot be opened. **This was corrected at the time of the inspection** -
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -1. In the room to the right of the entrance, there was a TV cord / other electrical cords dangling along a wall, and accessible for children. ** Please ensure the wires are not dangling or accessible for children. ** This was corrected at the time of the inspection. 2. In the room to the left of the entrance, there were some string lights installed around a cardboard structure where some sections were not secured and dangling/assessable to children. **Please ensure the string lights are not dangling or accessible for children. **This was corrected at the time of the inspection **3. Under the whiteboard, in the room to the right of the entrance, there was an electrical outlet without a proper safety cover. ** Ensure all outlets have safety covers installed if accessible to children. **This was corrected at the time of the inspection** 4. Some of the acrylic paints use for craft activities did not have the AP label or ASTM-4236 symbol on them.** Ensure all paint and glue products have the AP symbol or ASTM compliant writing on it , as those are products tested and proved to be nontoxic to children. ** Operator disposed of the products, and this was corrected at the time of the inspection ** -
    • 08. Is this facility properly constructed (Non-Critical)?
      • -1. There was a ceiling tile missing in the room to the left of the entrance. ** Install missing ceiling tile. 2. There were some water-stained ceiling tiles observed in the room to the left of the entrance, and in the hallway. ** Ensure the source of water infiltration had been properly repaired. Then, remove the water-stained ceiling tiles and replace with new tiles. Continue to monitor for additional staining. -
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • There were a few portions of paint along the wall that were worn. Repair the walls to ensure all surfaces are smooth, impervious to moisture, and easy to clean.

      • - 1. Around 4-5 dead bugs were observed on the floor in the washroom to the left of the entrance. ** Please clean the floors2. Some of the mats in the room to the left of the entrance, were damaged and the cushioning was exposed. **Remove the damaged mats if unused. Or, repair/replace the mats that have the cushioning exposed. Surfaces must be smooth/easy to clean. -
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • -Some harder to clean items - such as plastic dolls with artificial hair, were indicated to be sprayed with solution and allowed to air dry. ** Please ensure these items are laundered or first washed with soap/ water and then soaked in solution as there are many crevices + the artificial hair makes it hard for effective cleaning / disinfection throughout if just spraying. -

      • - The children jackets/ backpacks hanging on the hooks were overlapping and not separated. **Space out children's personal items so they are not overlapping or install dividers in between. -

      • -The fans in the washrooms were not operating properly as it did not pass the toilet paper test. ** Please repair or replace so the fans are operating properly.
