- Permit posted onsite expired May 31, 2025. Operator stated that the association email address was changed, and the permit might have been sent to the old email address.Operator will contact AHS permit admin on the change of email address and request for the current permit. Operator will send photo of the permit as soon as it is available.

- Observed some of the canned foods stored on the shelves in the storage room appeared dented, and a few missing food packaging labels. - Discussed with operator on the state of food items donated to the center to ensure the food is safe for consumption. - Operator will remove food cans that are dented, bulged or missing manufacturer's label.
- 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
- There were no chlorine test strips to check sanitizer concentration and to ensure utensils are adequately sanitized. Operator will order appropriate sanitizer test strips before the program commences.
