      • Observation (CORRECTED DURING INSPECTION): Found ice machine to have mildew/mould growing inside.
      • Corrective Action(s): A cleaning of the ice machine is required to remove all mould and mildew. Do not use ice until it is cleaned and sanitized.

      • Corrective Action(s): 1. Utensils should not be kept in sanitizer between uses as food may become contaminated with sanitizer. Use ice water and change as needed or leave in food itself.
      • 2. Ensure all food container lids are closed tight when not in use to prevent contamination of food by rodents
      • 3. All food should be store 6" or 15 cm off of the floor

      • Observation: Found mice droppings in walk in cooler under the shelving, in the dry storage on the bottom 2 shelves and on top of open containers, and upstairs behind and between all of the deep freezers.
      • Corrective Action(s): 1. remove all droppings, sanitize areas with chlorine bleach
      • 2. Monitor areas for new droppings, this will tell you where the mice are still coming in.
      • 3. Continue to follow direction of pest control company.
      • 4. Provide a copy of the last pest report to health department.

      • 2. Partition in walk in cooler is no longer in good condition as it is heavily worn creating space for mice to enter.
      • Corrective Action(s): 1. Ensure all holes are properly filled to prevent entrance and/or movement through the building.

      • Observation: Found mildew/mold growing on the ceiling in the walk in coolers and around the dishwasher. Floors in walk in coolers and dry storage upstairs require have mice droppings and debris accumulation. Walls/wire partition and shelving also have mildew growing on them.
      • Corrective Action(s): A deep clean of the floors, walls and ceilings in the above noted areas is required.

      • Observation (CORRECTED DURING INSPECTION): 1. Sanitizer spray bottles had 50 ppm quat, dispenser also had 50 ppm quat
      • 2. Ice machine has mildew starting to grow.
      • 3. Found multiple wiping rags on counter being reused and changed at end of shift.
      • Corrective Action(s): 1. Use quat test strips to manually make 200 ppm quat for spray bottles. Have sanitizer dispenser repaired or replaced so it dispenses 200 ppm quat at all times.
      • 2. Have ice machine cleaned and sanitized to remove mildew from inside.
      • 3. Keep wiping rags in sanitizer to prevent the spreading of pathogens, or change dry clothes frequently

      • 2. Found a few containers/trays placed in cooler/freezers without being covered.
      • Corrective Action(s): 1. Storing knives in sanitizer can lead to chemical contamination of food, or transfering of bacteria from drying cloth to knives. Store knives in either ice water changing frequently or change knives every couple of hours.
      • 2. Ensure all food is being covered either with lids or plastic wrap or metal foil to prevent the contaminataion of food.

      • Observation (CORRECTED DURING INSPECTION): Large container beef broth with yesterdays date was probed and had an internal temperature around 7.5C, ambient temperature of walk in cooler was 2.5C. Cooling process still not being completed with in 6 hours, as required. Improper cooling of foods is the number one cause of food borne illness.
      • Ice wands present on site and in use.
      • Corrective Action(s): Other methods to speed up the cooling process include breaking down hot item into smaller portions and/or putting onto shallow pans to allow heat to dissipate faster as well as using an ice or cold water bath. Review with staff the importance of cooling large items (including stock, soups, etc) as well as how to go about the chosen method of cooling.

      • Observation (CORRECTED DURING INSPECTION): French onion soup that was made yesterday stored in large clear container in the walk in cooler was 9.3C internally.
      • Food must be cooled from 60C to room temp in under 2 hours then from room temp to 4C or less in under 4 hours (6 hours total). This soup has far exceeded the time allowed for it to be 4-60C.
      • Corrective Action(s): Soup was voluntarily discarded during inspection. Ensure staff are using more than just time cooling on the counter to cool any large volumes of sauce/soup down. Additional methods to speed cooling include putting container of room temp sauce/soup in a cold water (or ice) bath stirring frequently or using shallow containers where food is no deeper than 2".

      • Observation (CORRECTED DURING INSPECTION): Bar glass washer had no residual sanitizer in rinse water. Chlorine bottle that was connected to bar glass washer was empty.
      • Corrective Action(s): Chlorine must be 100ppm in bar glass washers (not 50ppm due to low temperature of final rinse). Bottle switched out during inspection. Ensure this is checked daily so that the sanitizer does not run out when washer is in use.

      • Observation: 1)Three boxes of whole potatoes starting to turn green around handle of box. Boxes stored in main kitchen area and were marked as received six days ago. When potatoes turn green they contain a toxin that cannot be cooked out.
      • 2) Soap dispenser at hand wash station near the dishwashing area was located directly over a container of clean utensils
      • Corrective Action(s): 1) Remove green potatoes and either completely peel off green areas or discard. Recommend storing potato boxes in a dark room or covering to prevent light from touching them which causes them to start turning green. Due:today
      • 2) Do not store clean utensils under the soap dispenser as soap can get onto the utensils and then onto food. Container with utensils moved during inspection.

      • Observation (CORRECTED DURING INSPECTION): Observed two large whole cooked roasts in walk in cooler (large enough to just fit the 25L container that each was stored in). These roasts are too large to be able to cool properly within the required times (ie 60C to 20C in 2hours or less then from 20C to 4C or less in less than 4 hours).
      • Corrective Action(s): Before roasts are put into the cooler they must be cut down into smaller pieces or even fully sliced.

      • Observation: Hand wash station in the prep area lacked paper towel and the motion activated soap dispenser was not fully functional. Hand was station near dish washing area lacked accessible soap (dispenser may have been faulty)
