      • Corrective Action(s): Repair or replace the paper towel dispenser.
      • Write out instructions for the staff to show how to turn on the quat dispenser. Email me a copy of the instructions as this has happened on a previous inspection when the operator was not on site.

      • Ceiling lights were off when I entered the facility while food preparation was being done.
      • Corrective Action(s): Ceiling lights must be on at all times when in operation to provide the correct illumination at food preparation surfaces.
      • Repair hand sink. This is an outstanding item now recorded on 2 previous inspection reports.
