      • Observation:
      • Accumulation of mould/mildew found on the interior surfaces of the table-top ice making machine. Mould/mildew could contaminate the ice and could make patrons ill.
      • Corrective Action(s):
      • Clean the interior surfaces with hot, soapy water, and then sanitize. Add the cleaning of the interior surfaces of the ice making machine to the Cleaning Schedule, so it is done regularly.

      • Observation:
      • There is an open hole in the wall and floor junction, in the dry storage area under the racks storing canned soup. There are a few rodent droppings and gnawed material around the hole. This is indicating rodents are entering the premises through the hole. Rodents carry diseases.
      • Corrective Action(s):
      • Immediately block and seal the hole with a metal plate, wire mesh (1/4" or smaller), or tightly stuff with steel wool and then seal sith expanding foam. Clean up the droppings and sanitize surfaces that have come into contact with the droppings.

      • Observation:
      • 1) The exhaust canopy filters have accumulated grease and dust on them. Staff indicate that these are cleaned by the professional company every 6 months. The accumulation of grease is a fire hazard and it will attract pests.
      • 2) There are food stains on walls behind and below work tables. This will attract pests.
      • Corrective Action(s):
      • 1) Immediately wash and sanitize the exhaust canopy and filters. Add this duty to the cleaning schedule, so it is done on a regular basis (monthly). Provide the necessary time and equipment needed to wash and sanitize them safely and properly.

      • Dishwasher is constantly dripping water under the machine. Water is pooling under the dishwasher and this will breed flies and attract mice (water source).

      • Call a company to repair the dishwasher so it doesn't drip or leak water onto the floor.

      • There are missing wall tiles and window frame beside the dishwasher. The exposed drywall is unsealed and not cleanable. This will attract pests.

      • Repair the wall and window frame so the surfaces are durable, level, smooth, sealed and cleanable.

      • Observation:
      • Bait boxes containing rodent poison found in food storage and processing areas (kitchen). This is unsafe as the poison could be spread via rodent feet and hands when they touch it, and then walk elsewhere.
      • Corrective Action(s):
      • Immediately remove all bait boxes from food processing and storage areas. Only mechanical means of eliminating pests shall be utilized.

      • Observation:
      • 1) There are gaps at the base of each door leading out of the kitchen, to other areas of the facility. These gaps will allow pests to enter the kitchen.
      • 2) Interior and exterior garbage cans do not have tight fitting lids on them, to exclude pests. This will attract and breed pests.
      • Corrective Action(s):
      • 1) Install door guards or door sweeps at the base of each door leading out of the kitchen, to exclude pests entering.

      • 2) Purchase and place tight fitting lids on all exterior and interior garbage cans. The lids shall be kept on the garbage cans at all times, when not in use.

      • Observation:
      • 1) The high temperature dishwasher has broken temperature gauges and lights. Staff are unable to determine if the temperature is reaching the required levels for washing and sanitizing.
      • 2) The sink, in front of the dishwasher, is leaking and water is pooling on the floor. This will provide a water source to pests.
      • 3) The drain pipe, under the sink in front of the dishwasher, is leaking and water is pooling on the floor. This will provide a water source to pests.
      • 4) The clear plastic lids, of the dry storage bins storing oatmeal flakes and other dry goods, are cracked. These lids are not cleanable and bacteria will live in the cracks, and/or small pieces of plastic could fall into food.
      • Corrective Action(s):
      • 1) Repair or replace the temperature gauges and lights so they work correctly and accurate show the temperature of the washing and sanitizing water.

      • Reseal the floor in the mentioned areas so it is smooth, level, durable, and cleanable.

      • Observation (CORRECTED DURING INSPECTION): Dishwasher is not consistently reaching minimum water temperature for sanitizing dishware.
      • Corrective Action(s): Discussed temporary/emergency measures to follow until dishwasher repairs are completed:
      • Option 1) Visually-confirm wash temperature gauge is 75 degrees Celsius (or higher) prior to feeding each load of dishes into the machine.
      • Option 2) Manually-sanitize each load of dishware with a food-grade sanitizer solution (eg. 100ppm chlorine bleach/water solution) after washing in the dishwasher.
      • Option 3) Use single-service dishware/utensils for food-service until dishwasher repairs have been completed.
      • *Operator indicates that they plan to use option 1-->Ensure all dishwashing staff are informed of the emergency measure until dishwasher repairs have been completed.

      • Corrective Action(s): Disinfect and clean up the mouse droppings.

      • Observation: New evidence of rodent activity observed in the food storage room along the floor/wall junction previously filled with spray foam (scratch/chew marks in the spray foam)
      • Corrective Action(s): Re-seal the opening using a rodent-resistant material (eg. metal mesh, steel wool, etc.)

    • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): Dishwasher is not reaching sufficient rinse temperature for sanitizing dishware. Maximum temperature recorded after numerous cycles was 69.9 degrees Celsius, however, most cycles reached between 60 and 65 degrees Celsius. Incoming water temperature (max. temp was 45 degrees Celsius from the faucet) may not be sufficient in temperature to be boosted above 71 degrees Celsius by the booster heater.
      • Corrective Action(s): *Dishwasher repair technician was called to assess and repair (as required) the dishwashing machine by the manager.
      • *As a temporary measure, all dishware must be manually-sanitized using a sanitizer solution (eg. 100ppm chlorine bleach and water) until dishwasher adjustments/repairs have been completed.
      • *Demonstrated correct mixture ratio of chlorine bleach and water to the manager at time of inspection. Use 5 capfuls of LT Sanitizer product in the sink basin (filled to the drain plug overflow level) to achieve 100ppm concentration solution (minimum concentration required for manual sanitizing).

      • Observation: ouse droppings observed on metal wire food storage shelves near the walk-in cooler.
      • Mouse droppings observed on metal trolley shelf and on the floor underneath the trolley near the walk-in cooler. Orkin pest control report dated yesterday confirms mouse activity inside the storage room and around building exterior.
      • Corrective Action(s): *Thoroughly clean and disinfect surfaces/areas where mouse droppings are found. Cleaning staff should wear disposable gloves and masks when removing droppings for personal protection.
      • *Actively monitor all the food storage areas for new evidence of mouse activity (eg. droppings).
      • *Ensure to follow any/all recommendations from the pest control technician to mitigate/eliminate pest activity in the premises.

      • Observation: Final rinse temperature gauge on the dishwashing machine is not accurately displaying the actual temperatures inside the machine. Kitchen staff are relying on the temperature gauge to confirm that the dishwasher is reaching sufficient temperature for sanitizing dishware (ie. 180 degrees F).
      • Corrective Action(s): Re-calibrate or replace the final rinse temperature gauge to accurately display the temperatures reached inside the machine.
