- -A rug at the entrance in the common area was damaged from one of the sides/corners. This is posing a tripping hazard.The operator was advised to address this issue.

- -Safety belt was not observed for diaper changing surfaces.The operator was advised to acquire a safety belt.

- -Infant/toddler room: Contaminated rags used for cleaning the surfaces were lying on the shelf where children's tiffin boxes were stored.The operator was advised to keep the cleaning cloths in a sanitizer solution when not in use or use single-use paper towels to clean and disinfect all the surfaces.

- -The mouthed toys in the infant/toddler room were not stored separately to be cleaned and sanitized between each child's use.The operator was advised to keep a separate container for mouthed toys. The container should be properly labeled.

- -The toys were observed to be stored in the sinks after cleaning them. Ensure there is adequate space in the facility to store toys or remove the toys that are no longer required.

- -The alcohol-based hand rub was observed to be used past the expiry date. The operator was advised to acquire new ABHR for the facility.
