   • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
     • There is some exposed particleboard where arborite finish has chipped off piece of particleboard along bottom side of cabinet that has the coffee machine on top. Please repair or seal the particleboard.The countertop in staff kitchen area is chipped. Please repair it.
   • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
     • There is a leak from the walk-in-freezer drain line (main floor) creating some ice build-up on the floor in the walk-in-freezer. Repair leak so a slipping hazard is not created.
   • 23. Is the facility maintained in a clean and sanitary condition?
     • Overall cleanliness is very good, just a few areas still need cleaning: staff kitchen in some of the cabinets and basement dry storage area where some pieces of food have fallen onto the floor. Please clean these areas.
