      • Observation (CORRECTED DURING INSPECTION): Hot-held soups/chillies measured at internal temperature of 55'C during inspection. Staff last checked temperatures of hot-held food items less than 2 hours ago. Manager filled the hot-holding unit with hot water.
      • Corrective Action(s): Maintain all hot-holding units at or above 60'C at all times to prevent bacterial growth and/or toxin production. Measure food temperatures as per food safety plan with probe thermometer. Any food items which are in the danger zone (4'C-60'C) for more than 4 hours must be discarded.

      • Observation: Numerous small flies observed throughout main prep area in back kitchen, mostly between hand wash station and ice machine around the floor drain. Pest control company currently in once per month, their last report also noted fly activity and they recommended a hydro flush which has been done.
      • Corrective Action(s): Consult with pest control company and have areas of concern such as drains and other wet areas (ex grouting under dishwasher) treated for flies as the flushing of the drains did not fully remediate the problem. Follow recommendations of pest control company. Due: 2 weeks.

      • Observation:
      • Found food debris accumulating along the top edge of cupboard doors, cooler seals; on shelves inside lower cupboards; and on the inside surfaces of cooler doors. These areas will attract pests.
      • Corrective Action(s):
      • Wash with soap and water, rinse, and then sanitize these areas. Them mentioned areas should be part of your "detailed cleaning schedule" that require to be done daily, or at least once a week.

      • Found cracked and damaged plastic food inserts. Bacteria can breed in the cracks, and pieces of plastic could fall into food.

      • Discard any damage food inserts, lids or utensils. Ensure staff are inspecting all equipment, utensils, containers and lids for damage. During the inspection, all damage food inserts were discarded.

      • Observation:
      • Found plastic lids for food inserts/containers are cracked. Cracks allow germs to hide and multiply in them.
      • Corrective Action(s):
      • Throw away any cracked, chipped or heat damaged food container and/or lids. Purchase new ones, and ensure to inspect equipment on a regular basis for damage.

      • Observation:
      • Observed staff completing multiple tasks with the same pair of disposable gloves on. Disposable gloves will transfer germs from one task to food or another surface if they are not changed.
      • Corrective Action(s):
      • Immediately review the proper use of disposable gloves with all staff. Gloves shall be worn to complete one task, removed, and then thrown away. Hands MUST be washed prior to putting on a new pair of gloves. Do not allow staff to take the gloves off and "save them" for a later use, they must be thrown away immediately.

      • Observation:
      • Found drain fly problem in the premises, especially in the back food preparation area. Supervisor explained that Orkin has been contracted to eliminate the pest, blue lights and fly paper present in premises. Flies can transfer germs to food and/or could fall into food.
      • Corrective Action(s):
      • Increase the frequency of cleaning hard to reach areas and ensure staff are using hot water and soap to remove food stains, and then use sanitizer to kill the germs.

      • Observation:
      • General sanitation level is adequate, but food debris and pooling water found underneath and behind equipment; on pipes and walls beneath sinks; on legs of tables or equipment; and on the undersides of tables, equipment, shelves, and sinks. This will attract and breed pests (flies, cockroaches, etc.).
      • Corrective Action(s):
      • Detailed cleaning of the premises is needed on a daily basis to remove food debris. Review the proper method to cleaning all surfaces with staff. Soap MUST be used to remove food debris and sanitizer MUST be used to kill germs. Sanitizer isn't a degreaser and will NOT remove food debris. Provide extra time for staff to clean these areas, or have one staff member only complete the detailed cleaning during their shift.

      • Observation:
      • 1) Walk-in-freezer #1 has a broken door gasket. Ice is building up around the base of the door and then melting. Water then pools on the floor and creates an environment for pests to breed.
      • 2) The upright fridge, storing the bottled drinks (by walk-in-freezer #1), has pooling water underneath it. This will attract and breed pests.
      • Corrective Action(s):
      • 1) Replace the broken door gasket on the walk-in-freezer #1 door to stop cold air escaping and mixing with warm air, causing the ice to build up around the door.
      • **Correction date: Jan. 1/19**
      • 2) Mop up the pooling water and have the upright fridge serviced to determine where the pooling water is coming from.
