- Observation: Fruit flies observed throughout food premises. They are on most walls, on boxes, on shelves, on ceiling in some areas and in significant numbers.
- Corrective Action(s): *Reviewed recent pest control invoices with no mention of fruit fly control in recommendations! Previous health inspection reports identified fruit fly activity. Why is the pest control technician not providing comprehensive recommendations on this issue?
- *The largest population of flies observed around the rational oven, notably behind the unit. Floor drain noted behind unit.
- *Significant fruit fly populations observed around the dish sinks and grease trap lid. Area below upright cooler which covers half the grease trap lid.
- *Have pest control technician provide comprehensive recommendations to address "out of control" fruit fly infestation. Review action plan.

- Observation: P trap below hand sink in the back is leaking. Either it was repaired and is leaking again, or it was not properly fixed after last inspection. Noted on report dated May 22, 2024. Discussed with supervisor and a call for service has already been made, including for leaking taps at this sink and dishsink.
- Corrective Action(s): Fix and take picture and send to me to verify. The exisitng p trap looks to be in marginal condition. Replace with new.

- Corrective Action(s): Replace the ice scoop bucket. If underneath the soap dispenser, soap and dirty water may get on the ice scoop and subsequently contaminate drinking ice.

- Observation: Floor wall connections, baseboards, and wood cabinetry deteriorating and becoming more and more difficult to keep clean. Despite obvious effort of staff to clean difficult areas the general deterioration of the premises is making it increasingly difficult. Mouldy silicone seals around sinks, floor and baseboards evident.
- Corrective Action(s): Monitor ability to keep facility clean and food protected from contamination. Recommend developing a renovation plan for future tenant improvements.

- Observation: Three compartment sink set-up lack sink stoppers. Only one stopper available. Staff fill and empty sinks individually to accomodate single stopper. Limited space for adequate air drying also noted. Storage shelves are full of dishes and sink is full of cleaned dishes.
- Corrective Action(s): Immediately acquire three proper sink stops to perform proper dishwashing as discussed. Construct additional dish daying storage capabilities to ensure build-up of dishes in the sinks doesn't occur. Send picture of sink stoppers within 24 hours.

- Observation: A) Ant infestation observed on floor at donut glazing station, behind on floor.
- B) Fly maggot observed in area where ants observed.
- Noted significant accumulation of food debris in this area, including on top of the floor drain. Ants will be targeting the accumulations of food and sugar. Flies will target decaying matter to lay eggs for breeding.
- Corrective Action(s): This area of the premises must be thoroughly cleaned on a daily basis to ensure food source is eliminated for the pests to access and use.
- Pest Control technician must be called and controls implemented to eliminate the infestation. Get report and recommendations from Pest Control technician for review at follow up inspection date noted on report.

- Observation: Many corners and hard to reach areas of the floor and behind equipment in this facility observed dirty with accumulations of food debris, greases and organic matter beyond that which might normally accumulate on a daily basis.
- Corrective Action(s): Daily, weekly and/or scheduled cleaning practices must improve. Perform a complete reset cleaning. This will be reviewed at follow up inspection date noted. Make sure staff (daytime and nighy staff) are accountable for cleaning. Review cleaning practices with all staff. Discuss more at follow up inspection.

- Observation (CORRECTED DURING INSPECTION): One of the hot holding inserts measured 45 C (chicken soup). Manager explained the reheat takes place in warming unit over period of time, but the product should not be sold until it gets to termperature and is properly labelled.
- Corrective Action(s): Make sure re-heating soups and all hot held items in steam table follow Tim Hortons Food Safety Plan for this product. Confirm in email that re-heating processes are being followed. Send email to confirm processes for these products as discussed.

- Observation: Observed the following areas of premises excessively dirty:
- A) Donut prep station including shelving dolly, counter space, floor and space around drain;
- B) Area below and around handwash station in the back of the premises at dish sink;
- C) In general floor around equipment (coolers/counters etc.) in hard to reach areas;
- d) Walls with splattered food in many areas of premises and need to be cleaned up as it happens and not allowed to dry over multiple days.
- Corrective Action(s): Make sure staff, especially cleaning staff move all equpment that can be moved on a daily basis to ensure a thourough cleaning is completed and accumulations of food debris don't build up over multiple days. Aobve noted areas to be targeted for thourough cleaning. Day staff must also take responsibility for cleaning as it happens and not rely on cleaning staff to keep things clean. review staff cleaning procedures.

- Observation: Minor drain fly population noted at drive thru station. Noted visible areas where stagnant water building up as possible breeding habitat. Possible 'other' breeding areas not visible (behind, below and hiding in cavities of beverage service areas).
- Corrective Action(s): Improve cleaning and daily maintenance of potential breeding areas, especially below dish sink and around floor drain area behind donut trolley as discussed. Continue to have pest control target any potential breeding habitat including floor drains for pest prevention.

- Observation: A variety of areas of premises observed with accumulations of food debris. Also, there is the development of biomat slimes under dish sinks and around floor drain at donut prep station. Water observed leaking from valve in this area is also contributing to excessive moisture build up on the floor. This adds to the difficulty staff will have to properly clean. Trolley stored in this location (donut prep area) not adequately cleaned.
- Corrective Action(s): Improve cleaning practices in facility to ensure hard to reach and difficult to clean areas are given the proper attention to a complete cleaning, and then all areas of premises must be maintained better on a daily basis.
- All water leaks must be fixed and prevented from spreading to the floor and around the kitchen making cleaning difficult.

- Observation: Temperature of food products in the line/preparation cooler rangded from 10-11 C at the time of inspection (lunch rush??).
- All food products were transferred to the upright cooler operating at 4 C or below.
- Corrective Action(s): Repair or adjus the cooler so that it is capable of maintaining all food products at 4 C or below. Do not use this cooler until it is capable of maintaining acceptable temperatures. You may keep only the dressing etc in ice bath in the interim. ( immediately)
