- 4 dead mice were found within trap box. No signs of activity were observed outside of that.July 18, 2025 update:No mice activity was observed. When moving the egg station to check for mice activity, I observed two live adult cockroaches.I checked multiple glue boards but did not see any live or dead adults cockroaches or nymphs.

- The cutting boards at the sandwich station are stained and worn. Staff indicate it is difficult to clean. Replace or shave down cutting boards so they is smooth and easy to clean.

- REPEAT VIOLATION: One of the hand sinks had no soap.Ensure ALL hand sinks in the facility are properly supplied so staff can wash their hands at either hand sink. No installation of a wall pump is required, just put an individual soap pump at the hand sink to use.

- All hard-to-reach areas need cleaning and scrubbing, including walls, behind equipment, behind garbage bins, inside the cupboards. Floors and walls inside the area where the ice machine is located noted to have old spills and debris.Any unused equipment and other items are to be removed from the facility. Clean and tidy the back area to allow for proper cleaning and pest monitoring.
