- The cylindrical ceiling mounted fly trap close to the hot holding unit was filled with flies. Trap was removed during the inspection. Ensure fly traps are replaced as needed to protect food itemsensure equipment are effective to trap and kill flying insects and flies in the food establishment.

- Observed one muffin box on the floor in the walk-in freezer. Box was removed and placed on the shelve by staff.Ensure food boxes are store off the floor to prevent contamination.

- A few flies were observed in the food establishment. The fly traps in the two electronic insect killer were filled with dead flies. Thus, not effective to trap and kill flies at the time of inspection.Staff mentioned that the pest control company, Orkins was on site on Friday, July 14th.Replaced traps as needed to ensure equipment are effective to trap and kill flying insects and flies in the food establishment.
