      • Electrical outlet cover was missing in one of the outlets in the downstairs play space. Staff placed an outlet cover during inspection. Keep all outlets covered and eliminate any access to hazards for children.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • A green chair located in downstairs play room showed signs of wear and tear with paint chipping. Staff removed the chair during the inspection. Ensure that all surfaces are kept non-porous and easy to clean.

      • Diaper changing mat was not in good condition/ repair. Please make sure it is replaced.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • *Wooden tall doll house was not in good repair. Some of the parts were broken. Please make sure it is repaired, replaced or removed from the facility.*Soft play equipment, foam building blocks were not in good repair. Please make sure they are replaced or removed from the facility.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Upstairs couple of windows blinds had dangling cords. Please make sure secure dangling cords from blinds and other window coverings away from children’s reach.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Carpet on right corner upstairs by the small play area entrance was not in good repair; flooring material was damaged.Please make sure it is repaired.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Main floor - There were no CPR shields in the first aid kits. Please make sure CPR shields are available.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Small trampoline was noted upstairs room - Trampoline and re-bouncers are NOT permitted in social care facilities for ALL children.Please make sure it is removed from the facility.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • *Bleach water solution is used for toy washing but there was no Chlorine test strips available onsite. Please make sure Chlorine test strips are provided to main and upstairs to make sure sanitizer concentration are in required range for proper disinfection.*Big grey taped play tray upstairs room - was not in good repair. There were sharp points on the tray.Please make sure its repaired or replaced.Pine soil was also used as disinfectant the table surfaces. Operators dilute the product. It is cleaning product. Uncertainty of food grade or not, also multi surface cleaner- For a disinfectant it is required to have DIN number on. Kids eat their food on the tables, it is recommended to use food grade sanitizer, Quat 200ppm, or Bleach water solution 100ppm., or other alternative food grade sanitizer.
