- Corrective Action(s): iFx hot water tap as discussed. Recommend replacing taps with wing taps that may be more resilient to wear and tear. Check at next scheduled inspection.

- Observation: Basement ice machine observed with a significant amount of bioslime in and around ice making area. Inner ice compartment also looked to have scale or slime build up. Outer door edge also dirty.
- Corrective Action(s): Immediately stop using this machine. Discard all ice inside. Have unit claened and sanitized as discussed.
- Follow up inspection needed to observe cleaning. Send pics when cleaning done, if prior to re-inspection date.

- Observation: A) Handsink on the cookline has hot water and soap, but there is a sprayer on the end of the faucet that causes the water to spray everywhere, contaminating the area.
- B) Handsink downstairs missing soap in dispenser. Paper towel dispenser broken?? but alternate area for paper towel roll provided.
- C) Bar area lacks paper towels and the squeeze bottle soap dispenser is not ideal and not hands free.
- Corrective Action(s): A) Remove sprayers from handwash sink faucets.
- B) Replace soap in dispenser.
- C) Get paper towel dispenser with paper towels and find a more hands free soap dispenser.

- Observation (CORRECTED DURING INSPECTION): Window handsink has hot water turned off. manager stated due to advice from Pest Control if faucet leaking.

- Observation: A) Cookline handwash station is not maintained in a manner that staff can easily access and use. It has effectively been taken out of use in favour of the handwsh station in back area, and is not stocked with liquid soap and paper towels. Additionally, it poses a contamination risk to the tortilla chip station located along the window.
- B) Basement hand sink also not accessible and not in use. Paer towels not located in dispensing unit, and is instead located on shelving unit close to sinks. By default the two compartment sink co-located in this area is used for handwashing.
- Corrective Action(s): A) Cookline handsink must be brought back into use and the nacos station must be protected from contamination. Recommend re-location.
- B) The basement is in need of organizational changes, including making the handsink accessible. Additional and alternate storage space is needed.
- Review at follow up inspection date noted on report.

- Observation: A) Significant fly infestation noted in basement and to lesser extent at the top of the stairs to the basement. Several sticky tape traps in place in basement that were full of flies with more flying around.
- B) Mouse activity noted in the basement storage room, the dish pit area, and along floor wall connections in multiple areas of facility. Access and cleaning severly inhibited by storage and food supplies in multiple areas of facility.
- Corrective Action(s): Ai) Improve cleaning and remove debris that can attract flies is needed. Cover garbage and relocation of garbage and dirty cloths recommended.
- ii) Contact pest conrol contractor for advice and implement comprehensive fly control program immediately. There are two fly entry points from outside. Implement air barriers at doors to patio and main entry may be necessary if fly control cannot be established.
- B) Contact pest control contractor to increase rodent control in premises.
- **Provide plan of action to this office for both pest issues within two weeks.

- Observation: Dish pit area dirty - floor. Cubby area above stairs a mixture of clothing, food and equipment and is not clean. surfaces made of unfinished wood.
- Basement is a mess. It's dirty and disorganized. Storage room and office has boxes all over the place, making routine cleaning difficult. Floor and behind equipment is dirty. Walk-in cooler floor is dirty.
- Corrective Action(s): Clean up under and around noted areas.
- A new and different storage area is needed to allow many other parts of premises to stay organized and cleaned.
- Provide plan to this office on a plan to improve storage space. This may include construction of an additional storage space.

- Observation: Handwash stations in basement, cookline and bar observed with issues related to either access, lack of paper towels and/or liquid soap.
- **Cookline station looks to be turned off. Cookline handwashing provided soley by back room handwash station.
- Corrective Action(s): All handwash stations must be made available for use, and maintained as per original facility approval. This may require the relocation of food and/or shelves to make accessible so staff can use in efficient manner.
- Verify at reinspection date noted or send pictures by email showing completed work.

- Observation: Rodent activity noted (mouse droppings) to varying degrees throughout facility. More activity noted in areas where cleaning and organizational issues observed. (See other vilation).
- Corrective Action(s): Continue to work with Pest Control Contract to reduce mouse populations, AND target neglected areas of facility where there is a build up food debris.

- Corrective Action(s): Address cleaning in these noted areas to be more frequent or better quality. Move equipment as necessary to access food debris for removal. Send pics of completed work or have completed by follow up inspection date noted.
