- Observation(s): There is not a process in place to monitor food and nutrition services provided by the Licensee. Ensuring there is a process in place to monitor the food and nutrition services provided by the Licensee works towards promoting the health of persons in care. Submit by June 19, 2026, a written plan outlining how you will monitor the food and nutrition services provided to persons in care. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.

- R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Observation(s): Records of oral care provision are being maintained, however, there are no care plans outlining how much support and what type of support is needed, to maintain oral health. Lack of information guiding staff as to what oral care interventions are needed could lead to inconstant oral care provision for persons in care. Submit by June 19, 2026, a written plan outlining how you will ensure all persons in care have a oral health care plan. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.

- R6.2 - Have all persons admitted to the facility complied with the Province’s immunization and tuberculosis control programs?
- Observation(s): The current process for ensuring that all residents admitted to the facility have a record of TB screening appeared to be ineffective. Upon a random audit of records, Licensing Officer was unable to see evidence that TB screening had been conducted.

- R4.1 - Are person in care records current, complete and kept confidential?
- Observation(s): The current system in place to ensure that persons admitted to the facility have their height and weight recorded on admission appeared to be ineffective. Upon a random audit of records, Licensing Officer could not locate this information. In discussion with the Licensee, Licensing Officer confirmed that there is currently not a process in place to capture this information.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): The current system in place to ensure that any damage or needed repairs are conducted appeared to be ineffective. During the inspection, Licensing Officer noted numerous areas of the facility which had been identified by the Licensee to require repair, however, at the time of this inspection, there was not a plan in place to address the identified areas.
